宫腔镜联合B超检查诊断异常子宫出血104例临床分析

目的探讨宫腔镜联合B超检查在各种异常子宫出血中的诊断价值。方法我院应用宫腔镜联合B超检查异常子宫出血104例，术前均行B超检查，术中行定位取材或诊断性刮宫。结果在异常子宫出血病因诊断中，宫腔镜联合B超检查诊断子宫内膜增生症12例，与病理诊断的符合率为70．59％；子宫内膜息肉26例，符合率为92．86％；子宫黏膜下肌瘤13例，符合率为92．86％；慢性子宫内膜炎30例，符合率为93．75％。结论对异常子宫出血的病因诊断中，宫腔镜联合B超检查提高子宫内膜息肉及子宫黏膜下肌瘤的诊断率，尽管对子宫内膜增生症及子宫内膜癌需经病理诊断，但因镜下可以定位诊刮，亦提高了诊断率。宫腔镜联合B超检查是诊断各种异常子宫出血的有效且有价值的方法。

Hysteroscopy combined with B-type ultrasonography in diagnosis of abnormal uterinebleeding in 104 cases

Objective To discuss and evaluate the diagnostic value of hysteroscopy combined with B - type uhrasonography in determining the causes of various abnormal uterine bleeding. Methods 104 cases of abnormal uterine bleeding were examined by hysteroscopy combined with B - type ultrasonography in this hospital. B - type ultrasonic examination was applied before operation, localized tissue sampling and diagnostic uterine curettage were carried out during operation. Results Among them, endometrial hyperplasia was diagnosed by combination of hysteroscopy and B - type ultrasonic examination in 12 cases, endometrial polyps in 26 cases, submucosal myoma of uterus in 13 cases and chronic endometritis in 30 cases, and the coincidence rates with results of pathological diagnosis were 70.59%, 92.86%, 92.86% and 93.75% respectively. Conclusion In diagnosis of abnormal uterine bleeding, the combination of hysteroscopy and B - type uhrasonography can raise the diagnostic accuracy of endometrial polyps and submucosal myoma of uterus. The diagnosis of endometrial hyperplasia and endometrial careinoma requires pathological diagnosis, and hysteroscopy can improve the diagnostic accuracy by permitting localized curettage. The examination of hysteroscopy combined with B ultrasonography is an effective and valuable method for etiological diagnosis of abnormal uterine bleeding.
