摘    要:慢性肾脏病(chronic kidney diseases,CKD)患者常伴发脂代谢异常,同时多种与脂代谢异常有关的疾病如糖尿病、高血压亦常合并CKD。尽管人们认为血脂异常可能是由肾脏疾病引发的,但大量的临床和实验研究支持脂代谢紊乱会导致肾脏疾病的发生并加速其恶化。高脂血症在诱导肾损害中起着重要作用并且是CKD进展的独立危险因素。虽然脂质诱导肾损害的潜在病理生理机制尚未被阐明,但一些研究提示了脂蛋白、甘油三脂、胆固醇和游离脂肪酸影响肾小球和肾小管功能的新机制。作者将重点介绍高脂血症在CKD中致病机制的研究进展。

Abstract:Chronic kidney diseases (CKD) patients have abnormal lipid metabolism, and a variety of diseases associated with abnormal lipid metabolism such as diabetes and hypertension are combined with CKD. Although dyslipidemia may be thought to be caused by kidney disease, a large number of clinical and experimental studies support the disorder of lipid metabolism leading to kidney disease and accelerating its progression. Hyperlipidemia plays an important role in the induction of renal damage and is an independent risk factor for the progression of CKD. Although the underlying pathophysiological mechanisms of lipid-induced renal damage have not been elucidated, some studies suggest that lipoproteins, triglyceride, cholesterol, and free fatty acid affect kidneys through new mechanism of glomerular and tubular function. This review will focus on the research progress of the pathogenesis of hyperlipidemia in CKD.
