肾衰合剂对肾盂内高压灌流致肾结石患者肾单位损伤的临床疗效研究

目的探讨肾衰合剂对肾盂内高压灌流致肾结石患者肾单位损伤的疗效及对肾功能的影响。方法选择该院2017-01~2018-10收治的92例肾结石患者,采用随机数字表法分为对照组和观察组,每组46例。所有患者采用经皮肾镜取石术治疗,对照组术后给予常规抗炎治疗,观察组在此基础上予以肾衰合剂治疗。比较两组结石清除成功率、术后并发症,治疗前、术后2个月血清肌酐(Scr)、尿素氮(BUN)及尿中性粒细胞明胶酶相关脂质运载蛋白(uNGAL)、β2微球蛋白(β2-MG)水平的变化情况。结果对照组清石成功率为93.49%,观察组为95.65%;对照组术后并发症发生率为8.70%,观察组为6.51%,两组比较差异均无统计学意义(P>0.05)。观察组术后2个月Scr、BUN、uNGAL和β2-MG水平低于对照组(P<0.05)。结论与对照组比较,肾衰合剂可降低肾结石术后肾盂内高压灌流肾单位损伤患者Scr、BUN水平,改善肾功能,临床疗效满意。

Clinical study of Shenshuai Mixture in treatment of nephron injury caused by high pressure perfusion in renal pelvis in kidney stone patients

Objective To investigate the clinical efficacy of with Shenshuai Mixture in treatment of nephron injury caused by high pressure perfusion in renal pelvis in kidney stone patients and its effect on renal function.Methods Ninety-two patients with renal calculi admitted to our hospital from January 2017 to October 2018 were randomly divided into control group and observation group by random number table method,with 46 cases in each group.All the patients were treated with percutaneous nephrolithotripsy.The control group was treated with routine treatment of anti-inflammation after operation and the observation group was treated with the same treatment as the control group plus Shenshuai Mixture.The success rate of stone clearance and postoperative complications were compared between the two groups.The changes of the levels of serum creatinine(Scr),blood urea nitrogen(BUN),urine neutrophil gelatinase-associated lipocalin(uNGAL)and beta-2-microglobulin(β2-MG)were compared before and 2 months after operation.Results The success rate of stone clearance was 93.49% in the control group and 95.65%in the observation group,and there was no significant difference in the incidence of complications after operation between the control group(8.70%)and the observation group(6.51%)(P>0.05).The levels of Scr,BUN,uNGAL and β2-MG in the observation group were significantly lower than those in the control group 2 months after operation(P<0.05).Conclusion Compared with the control group,Shenshuai Mixture can reduce the Scr and BUN indexes of the patients with nephron injury caused by high pressure perfusion in renal pelvis after nephrolithiasis operation,improve the renal function,and the clinical effect is satisfactory.
