| 摘 要： | 目的：探讨重症胰腺炎（SAP）术后并发症的发病因素以及预防和治疗措施。方法：回顾分析1990-04～2009-02收治经外科手术治疗的87例SAP临床资料。术后发生严重并发症32例（36.8%）,死亡17例（19.5%）。结果：SAP术后有较高的并发症发生率和病死率。多个器官损害比一个器官损害死亡率有非常显著的统计学意义（P〈0.01）。因呼吸功能不全、功能衰竭导致ARDS死亡6例;腹腔室膈综合征（ACS）死亡4例;胰性脑病（PE）、多器官功能衰竭（MOF）死亡3例;十二指肠瘘;肝、肾功能衰竭各死亡2例。余临床治愈出院。结论：ARDS、ACS、继发感染是SAP死亡的主要原因。影响预后的主要因素是并发症的早期预防措施和治疗方法。采用个体化治疗相结合的原则是影响手术病死率与抢救成功率的重要因素。

| Abstract: | Objective:To investigate the risk factors of postoperative complications and the methods of prevention and treatment of severe acute pancreatitis(SAP).Methods: An outcome-based retrospective review of 87 patients who had received the operation therapy on SAP during Apr 1990 to Feb 2009 was performed.Severe postoperative complication happened in 32 patients(36.8%).Of these 32 patients 17(19.5% of the total)died.Results: There was a high occurring rate of postoperative complication and a high mortality for SA... |
