鼻内镜下脑脊液鼻漏修补术39例分析

目的探讨鼻内镜下脑脊液鼻漏修补术及体会。方法回顾性分析2008年7月～2010年7月39例脑脊液鼻漏患者病例资料,其中骨折发生在筛窦18例,蝶窦12例,额窦9例;瘘口大小5mm×5mm～30mm×30mm。经鼻内镜找到漏口,搔刮创面后用自体材料修补,覆盖筋膜,或填塞自体肌肉封闭窦腔。结果随访2个月至2年,39例均治愈,平均治愈时间16d。结论鼻内镜下脑脊液鼻漏修补术是外科治疗脑脊液鼻漏的首选方法。

Clinical analysis of endoscopic repair of cerebrospinal fluid rhinorrhea in 39 cases

Objective To explore the repair of cerebrospinal fluid rhinorrhea under nasal endoscope and to summarize the clinical experience.Methods Clinical data of 39 cases with cerebrospinal fluid rhinorrhea accepted endoscopic repair from July 2008 to July 2010 was studied retrospectively.The fracture was located in thmoid sinus in 18 cases,sphenoid sinus in 12 cases and frontal sinus in 9 cases.Fistula size ranged from5 mm×5 mm to 30 mm×30 mm.After finding the leak site under endoscope,the wound was scratched,repaired with autologous material,covered with fascia and filled or enclosed the sinus with autologous muscle.Results All the 39 patients were followed up for 2 months to 2 years and got cured,with an average healing time of 16 days.Conclusion Endoscopic repair is the first choice for the cerebrospinal fluid rhinorrhea.
