多层螺旋CT在急腹症上消化道穿孔中的应用

目的:评价多层螺旋CT(MSCT)在急腹症上消化道穿孔中的临床应用价值.方法:选取经确诊为上消化道穿孔患者80例的MSCT资料进行分析,探讨急腹症上消化道穿孔的MSCT特点.结果:80例患者中,MSCT诊断出上消化道穿孔共79例,占98.8%.其中78例患者表现为单个或者多个腹腔内游离气体,多为新月形、半弧形甚至游离小气泡,位置以镰状韧带周边、肝门周围、圆韧带周围以及膈下为主,1例初始表现为腹腔积液,呈带状分布于胃穿孔部位,经治疗一周后再行MSCT发现腹腔内游离气体.结论:MSCT在在急腹症上消化道穿孔的诊断中起到定性诊断的作用.

Application of multi-slice spiral CT in upper gastrointestinal perforation of acute abdomen

Objective To evaluate the clinical value of multi-slice spiral CT (MSCT) in the upper gastrointestinal perfora-tion of acute abdomen. Methods The MSCT data of 80 cases diagnosed as upper gastrointestinal perforation were analyzed, To investigate the MSCT features of upper gastrointestinal perforation in acute abdomen. Results Of the 80 patients, MSCT diag-nosed 79 cases of upper gastrointestinal perforation, accounting for 98.8%. Of these, 78 patients showed single or multiple intra peritoneal free gas, Mostly crescent, half arc, or even small bubbles, The location is mainly around the periphery of the sickle ligament, around the hepatic hilum, around the round ligament, and under the diaphragm, The initial manifestation of 1 cases was peritoneal effusion, Banded to the site of gastric perforation, After treatment for one week, MSCT was used to detect the free gas in the abdominal cavity. Conclusion MSCT plays an important role in the qualitative diagnosis of upper gastrointestinal perforation in acute abdomen.
