腹腔镜消化道内镜联合治疗胰胆管病

目的　探讨胰胆管病的腹腔镜 -消化道内镜联合治疗方案。方法　选择腹腔镜、十二指肠镜、胆道镜中 2镜或 3镜有机联合 ,治疗胰胆管病 2 0 0 5例 ,包括胆囊结石合并继发性胆总管结石1 2 82例 ,原发性胆总管结石 32 3例 ,肝内胆管结石 1 0 6例 ,肝内、外胆管结石 2 5 1例 ,慢性胰腺炎 6例 ,Mirizzi综合征 2 4例 ,先天性胆总管囊肿 (Ⅰ型 ) 1例 ;恶性梗阻性黄疸 1 2例。结果　各治疗方案较传统开腹手术创伤轻、痛苦小、手术并发症少。结论　腹腔镜 -消化道内镜联合治疗胰胆管病 ,达到了微创外科的效果

The combined treatment programs of laparoscopy and endoscopy for pancreo-cholepathia

Objective To study the combined programs of laparoscopy and endoscopy for pancreo cholepathia.Methods The two or three among laparoscope,duodenoscope and choledochoscope were combined to treat 2005 cases of pancreo cholepathia including 1282 cases of cholecystolithiasis with secondary choledocholith,323 cases of primary choledocholith,106 cases of calculus of intrahepatic duct,251 cases of calculus of extrahepatic duct,6 cases of chronic pancreatitis,24 cases of Mirizzi syndrome,one case of congenital cyst of common bile duct,and 12 cases of malignant obstructive jaundice. The programs had minor trauma,mild suffering,and less operative complications,compared with traditional open operations.Conclusion The combined treatment of laparoscopy and endoscopy for pancreo cholepathia could produce the results of minimally invasive surgery.
