社区健康干预模式探讨

目的探讨社区居民健康状况及社区健康干预模式的建立。方法采用整群抽样方法,从全区16个居委会中各抽查100户,采取统一格式的调查表进行健康状况调查,并将结果录入计算机进行信息分析。根据初次调查结果,采取针对性健康教育、签订"家庭健康契约服务"合约、建立"双向转诊"绿色通道、成立"社区病友联谊会"等干预措施。结果慢性病得到不同程度的控制;72.35%(2355/3255)不同程度地纠正了不良行为和生活习惯;社区居民健康知识知晓率明显提高。结论在社区开展针对性健康教育对行为及危险因素的干预是防治慢性病的有效措施。

ANALYSIS ON THE METHOD AND EFFECT OF HEALTH INTERVENING AT COMMUNITY AREA

Objective Discuss the health status of the community inhabitants and the methods of health intervening in community area.Methods Group sampling.Select 100 households per community units,totally 16 community units in the given district,conduct the survey of their health status according to the formatted questionnaire and finally analyse the collected data through computer.Based on the result of the primary investigation,carry out the following preventive methods: pertinent health education,sign agreement of "household health contractual service",open green channels for "two-way shifting-diagnosis" and establish sodality for patients at community area.Results effectively control the chronic diseases in different degree separately,correct improper behaviors and living habits by 72.55%(2355/3255),markedly improve the community inhabitants' health knowledge.Conclusions Health education and perilous factors preventions are some of the most effective approaches in the control and prevention of chronic disease in community area.
