应用双腔Power经外周中心静脉置管与深静脉导管的舒适度及活动度比较

目的比较双腔Power经外周中心静脉置管(peripherally inserted central catheter,PICC)与深静脉导管的患者舒适度及活动度,探究更有益于患者舒适的置管方式。方法便利抽样选取2015年1-7月入住北京协和医院外科确诊为胰腺癌并接受手术治疗的患者96例为研究对象,其中:耐高压注射型双腔Power PICC组50例,深静脉组46例。两组均采用美国国立卫生研究所制定的视觉模拟评分法~(1])让患者进行舒适度的自评价,同时比较两种不同置管方式对患者输液部位活动度的影响。结果耐高压型双腔Power PICC组患者舒适程度高于深静脉导管组(P0.05);耐高压型双腔Power PICC患者置管后输液部位在输液时活动良好者占84%、非输液时占96%,而深静脉导管置管后输液时活动良好者仅有54.3%、非输液时占71%,整体上耐高压型双腔Power PICC组患者输液部位活动度明显优于深静脉导管组(P0.05)。结论耐高压注射型双腔Power PICC在舒适度及活动度方面优于留置深静脉导管,值得临床推广;同时,在管路维护过程中须加强对置管患者的舒适护理,优化护理流程,提高护理质量,提升患者满意度。

Comparison of Comfort and Activity between Double Cavity Power PICC and Central Venous Catheterization

Objective To compare the comfort and activity in the patients with double cavity Power pe-ripherally inserted central catheter (PICC)or central venous catheterization,and to explore the most com-fort way of catheterization.Methods By using case-control trial method,50 cases of double cavity Power PICC and 46 cases of central venous catheterization were selected and their self-evaluated comfort level was conducted by visual analogue scale.The effect of two different catheterization method on the degree of ac-tivity was compared.Results The comfort level in double cavity Power PICC group was higher than cen-tral venous catheterization group (P < 0.05 ).The good degree of activity in double cavity Power PICC group was accounted for 84% during transfusion,while 96% in non-transfusion time.The good degree of activity in central venous catheterization group was accounted for 54.3% during transfusion,while 71% in non-transfusion time.In general,the infusion site activity of the patients with double cavity Power PICC was better than that of central venous catheterization group (P<0.05).Conclusions The comfort level and degree of activity in patients with double cavity Power PICC are both higher than patients with central ve-nous catheterization,which is worth for clinical trial.In the mean time,it is necessary to strengthen the comfort nursing,improve the nursing process and nursing quality,as well as improve patients’satisfac-tion degree.
