产后母婴皮肤接触时间对初产妇住院期间母乳喂养的影响

目的探讨母婴皮肤接触时间对初产妇及健康足月儿住院期间母乳喂养的影响,为进一步开展母婴皮肤接触提供依据。方法采用便利抽样法选择2015年6-9月在同济大学附属第一妇婴保健院住院分娩的71例产妇为研究对象,按随机数字表法将其分为观察组(36例)和对照组(35例)。观察组产妇分娩后即刻给予母婴皮肤接触1h,对照组产妇分娩后给予母婴皮肤接触至会阴伤口缝合完毕,约20~30min,评价并比较两组产妇的新生儿首次母乳喂养吸吮时间、首次母乳喂养情况、产后第1天新生儿有效吸吮次数、出院当天泌乳量及出院时的喂养方式。结果观察组新生儿的首次母乳喂养吸吮时间为(17.06±8.66)min,长于对照组的(12.11±9.06)min,差异有统计学意义(t=2.057,P0.05);观察组产妇的首次母乳喂养成功率、产后第1天新生儿有效吸吮次数、出院当天泌乳量及出院时的纯母乳喂养率均优于对照组,差异均有统计学意义(P0.05或P0.01)。结论产后持续1h的母婴皮肤接触有利于促进初产妇住院期间的母乳喂养,值得在临床推广应用。

Effects of Different Time of Skin-to-Skin Contact after Birth on Breastfeeding among Primiparae during Hospitalization

Objective To explore the effects of different time of skin-to-skin contact after birth on breast-feeding among primiparae during hospitalization,and in order to provide evidence for the clinical nursing practice.Methods group (n=36)and control group (n=35).In the observation group,the newborns were given skin-to-skin contact for 1 hour,and the mother-infant dyads'skin-to-skin contact lased for 20-30 minutes,until the per-ineum was sutured.The indexes of the neonates'sucking duration of the first breastfeeding,the first breastfeeding successful rate,the effective sucking times in the first day,the amount of lactation and the exclusive breastfeeding rate at discharge.Results The neonates'sucking duration of the first breastfeed-ing in the observation group was (17.06±8.66)min,which was significantly longer than those in the con-trol group of (12.11±9.06)min (t=2.057,P<0.05).The first breastfeeding successful rate,the effective sucking times in the first day and the amount of lactation and the exclusive breastfeeding rate at discharge in observation group were significantly better than control group (P<0.05 or P<0.01 ).Conclusion Mother-infant skin-to-skin contact for 1 hour could promote breastfeeding effectively,and worth populari-zing in the clinical nursing practice.
