摘    要:目的观察中药大黄对脑外伤并发急性肾衰(ARF)患者近端肾小管功能的影响。方法观察组65例患者在常规治疗基础上给予大黄煎液,对照组50例患者单纯给予常规治疗;观察患者尿β2-微球蛋白(β2-MG)及α1-微球蛋白10(α1-MG)含量,同时观测尿素氮(BUN)及血肌酐(Scr)的变化。结果治疗后两组患者尿β2-MG及α1-MG呈不同程度下降;血BUN及Scr的变化与尿152-MG及α1-MG平行;治疗后上述4项指标均以观察组低于对照组(P〈0.05)。以Scr为自变量,β2-MG及α1-MG均与Scr呈正相关性(分别为rp=938,P=0.001;rp=965,P=0.000)。结论大黄可能是通过改善近端肾小管重吸收功能,促进恢复肾小管的修复,从而发挥治疗ARF的作用。

Abstract:Objective To observe the effect of rhubarb on the injury reabsorption function of proximal renal tubular acidosis (PRTA) in acute renal failure patients caused by cerebral trauma. Methods 65cases in the observation group were given rhubarb combined with regulatory treatment, whereas the ether 50 cases in the control group just with routine treatment. The urinary β2-microglobulin (β2-MG) and α-microglobulin ( α- MG) were detected and compared after a 4-week treatment, so were the serum urea nitrogen(BUN) and creatinine(Scr). Results Both β2-MG and α-MG downgraded in various degree, so did the BUN and Scr. The upper 4 indexes in observation were significantly lower than that of the control ones at the end of gth week ( P 〈 0. 05). About the correlation analysis, the urinary β2MG and α-MG presented positive correlation with Scr (rp=938,P=0.001;rp=965,P=0. 000 respectively). Conclusion Rhubarb can promote the recovery of reabserption function of proximal renal tubular acidosis in acute renal failure patients caused by cerebral trauma.
