大骨瓣开颅改良减压术治疗重型颅脑损伤的疗效分析

目的探讨大骨瓣开颅改良减压术治疗重型颅脑损伤的临床疗效。方法将91例应用标准大骨瓣开颅术治疗的重型颅脑损伤患者，随机分成大骨瓣减压术组（A组46例）和改良减压术组（B组45例），对临床疗效及并发症进行统计学分析。结果2组手术疗效差异无统计学意义（P〉0．05），改良减压术组术后并发症明显降低（P〈0．05）。结论大骨瓣开颅改良减压术治疗重型颅脑损伤疗效确切，能有效减少大骨瓣减压术的并发症发生。

Curative Effect of Large Trauma Craniotomy and Improved Decompression in Patients with Severe Craniocerebral Injury

Objective To discuss clinical curative effect of large trauma craniotomy and improved decompression in patients with severe craniocerebral injury（CI）. Methods 91 cases with severe CI that will undergo large trauma craniotomy decompression operation were divided into 2 groups randomly. Group A （n = 46 ） treated by standard large trauma craniotomy and decom- pression, and Group B treated by improved decompression. The difference of curative effect and complications were analyzed statistically, Results There was no obvious distinction between the two groups in surgical curative effect （ P 〉 0.05 ）. The postoperative complication decreased obviously in group B （ P 〈 0. 05 ）. Conclusions The curative effect of large trauma craniotomy and improved decompression surgery was confirmed for severe CI, It can reduce effectively the complications caused bv the surtgerv.
