参附注射液对肠缺血-再灌注大鼠肺损伤的影响

摘    要:目的探讨参附注射液(SF)对肠缺血-再灌注(ⅡR)所致肺损伤的防治作用及其对肿瘤坏死因子-α(TNF-α)、诱生型一氧化氮合酶(iNOS)和细胞间黏附分子-Ⅰ(ⅠCAM-1)的影响,及其防治肠源性肺损伤机制。方法SD大鼠随机分为缺血再灌注组、假手术组、SF组并予相应处理。以病理学改变作为评价肠、肺损伤的指标;监测再灌注前后平均动脉压变化;ELISA法检测血浆、肺组织TNF-α含量;用免疫组织化学方法检测肺及小肠组织iNOS和ICAM-1蛋白的表达。结果SF可明显减轻IIR导致的肺和小肠组织的病理损害,抑制IIR后出现的血浆及肺组织TNF-α水平升高,肺及小肠组织iNOS和ICAM-1蛋白表达的增加;再灌注后SF组平均动脉压变化较缺血再灌注组明显减轻。结论SF有防止肠源性肺损伤的发生,进而预防组织病程向多器官功能不全综合征发展的重要作用,这种作用可能通过抑制TNF-α、ⅠCAM-1和iNOS等介质的释放而实现。

Abstract:Objective: To explore the mechanism of Shenfu Injection(SF)' s protective effect on lung injury induced by intestinal ischemia- reperfusion(IIR) in rats. Methods: Adult Sprague- Dawley rats were divided into three groups: IR group, sham group and SF group, Pathologic methods were used to evaluate the degree of lung and small intestine injury. Plasma and pulmonary tumor necrosis factor-α(TNF -α) levels in each group were measured by ELISA, in addition, the expression levels and distribution of inducible nitric oxide synthase(iNOS) and intercellular adhesion molecule- I(ICAM- 1) in small intestine and lung tissues were determined by immuno- histochemical method. ResuLts: SF could significantly alleviate the pathologic lesion of lung and small intestine and improve the hypotensive status caused by IIR. It could also decrease the elevation of plasma and pulmonary TNF - α levels after IIR. Meanwhile, SF could markedly decrease the high expression levels of iNOS and ICAM - 1 after IIR. Conclusion: The result indicate that pretreatment with SF can prevent lung injury following IIR in rats, and such protective effect may be related to the inhibition of TNF - α, iNOS and ICAM - 1 expression.
