经腮腺前嚼肌径路治疗下颌骨髁突骨折的护理配合

目的探讨经腮腺前嚼肌径路治疗下颌骨髁突骨折的护理配合方法。方法回顾性分析2008年1月～2010年6月在本院经腮腺前嚼肌径路治疗下颌骨髁突骨折17例患者的临床资料,并总结护理配合要点。结果患者术程顺利,手术时间1.0～3.5h,平均(1.5±0.3)h;术后伤口愈合良好,面部对称性好;全部患者获得了良好的咬合关系,张口度恢复至>3.7cm。术后1个月复查X光片显示骨折对位和对线满意;术后3个月检查面神经均无功能障碍。结论经腮腺前嚼肌径路治疗下颌骨髁突骨折是一项精细的手术,手术室护士应加强术前访视,做好患者的心理护理;同时熟悉面部的解剖结构,术中准确及时传递器械,以保证患者获得满意的治疗效果。

经腮腺前嚼肌径路治疗下颌骨髁突骨折的护理配合

Objective To investigate the methods of nursing cooperation during open reduction and internal fixation for mandibular condylar process fractures through a pre;parotid transmassetiric approach.Methods A retrospective analysis of the clinical data of 17 cases with mandibular condylar process fractures in our hospital between January 2008 and June 2010 was carried out and the experience in nursing cooperation was summarized.Results All the surgeries were completed within 1.0 ～ 3.5 h,with a mean ± SD of（1.5 ± 0.3）h.The incisions healed well and all faces were in good symmetry.One week after surgery,occlusal relationship of most patients were recovered effectively,and the width of mouth opening was 2.5 ～ 3.5 cm,averaged 2.8cm at discharge.One to three months after mouth opening training,the width of mouth opening in all the patients improved to more than 3.7 cm.Postoperatively,the nasolabial groove in six patients shallowed but all recovered well within one month.One month after surgery,X;ray examinations showed satisfactory fracture apposition and alignment in all patients.Three months after surgery,there were no facial nerve dysfunction detected in all patients.Conclusions Treatment to madibular process fracture through pre;parotid transmasseteric approach is effective.All the nurses in the operation room should prepare thoroughly and all the articles for operations should be disinfected and sterilized strictly.Routine preoperative visit should be performed well to understand the patient＇s conditions and psychological care should be done as well.Strict aseptic procedure and cooperation with surgeons are the necessity for successful surgery.The health care education after surgery is also mandatory for patient＇s satisfaction.
