甲泼尼龙冲击治疗急性脊髓炎的临床分析

目的观察甲泼尼龙冲击治疗急性脊髓炎的临床效果。方法选取本院收治的急性脊髓炎患者36例,随机分为2组：观察组24例给予甲泼尼龙静滴5 d,随后口服泼尼松并逐渐减量直至停药;对照组12例给予地塞米松静滴10 d,随后口服强的松并逐渐减量直至停药,比较2组脊髓神经功能恢复所需时间及总体疗效。结果观察组总有效率（83.3%）显著高于对照组（50.0%）,并发症发生率（8.3%）低于对照组（33.3%）,差异有统计学意义（P〈0.05）,观察组患者自行下地行走、排尿恢复以及肌力改善2级以上所需时间均短于对照组,差异有统计学意义（P〈0.05或P〈0.01）。结论在临床上采用甲泼尼龙冲击治疗急性脊髓炎的疗效更好,也更安全,值得临床大力推广。

Clinical analysis of pulse-dose therapy of methylprednisolone on acute myelitis patients

Objective To observe the clinical efficacy of pulse-dose therapy of methylprednisolone on acute myelitis.Methods Thirty-six acute myelitis patients from our hospital were selected and divided into observation group（n=24） and control group（n=12）.The observation group received methylprednisolone vein injection for 5 d,and then oral prednisone,gradually reduced until withdrawal.The control group received dexamethasone vein injection for 10 d,and then oral prednisone,gradually reduced until withdrawal.The Spinal neurological recovery time required and the clinical efficacy of the two groups were compared.Results The total efficacy rate of the observation group（83.3%） was obviously higher than that of control group（50.0%）,and the rate of adverse reactions（8.3%） of observation group was lower than that of control group（33.3%）（P〈0.05）.The time of walk by oneself,micturition recovery,and muscle strength improved to grade 2 or more in the observation group were shorter than that in the control group（P〈0.05 or P〈0.01）.Conclusion The pulse-dose therapy of methylprednisolone on acute myelitis is effective and safe.It′s worthy of clinical promotion.
