乙肝疫苗联合乙肝免疫球蛋白阻断乙肝病毒母婴传播的临床效果

摘    要:目的:探讨乙肝免疫球蛋白(HBIG)和乙肝疫苗联合阻断乙肝病毒(HBV)母婴传播的疗效。方法:选择2014年12月~2015年12月间246例HBsAg阳性孕妇为研究对象,根据随机数字表法分为观察组及对照组,均123例。对照组仅新生儿单独接种乙肝疫苗,观察组孕妇及新生儿均联合接种乙肝疫苗和HBIG。结果:对照组及观察组新生儿中出现宫内HBV感染率分别为22.76%和9.76%,一年后对照组及观察组新生儿中HBV感染率分别16.26%和1.63%,两组间差异均存在统计学意义。对照组及观察组新生儿出生时及一年后对应HBsAb阳性率分别为8.13%、76.42%和27.64%、94.31%,两组差异存在统计学意义。结论:孕妇及新生儿乙肝免疫球蛋白和乙肝疫苗联合应用能够有效降低HBsAg阳性情况患者所产新生儿HBV感染率,同时能使新生儿对应的HBsAb转阳率有效提高,最终使HBV的传播阻断率提高。

The clinical effect of mother infant transmission of hepatitis B virus hepatitis B vaccine combined with hepatitis B immunoglobulin blocking
Abstract:Objective To investigate the effect of hepatitis B immunoglobulin (HBIG) combined with hepatitis B vaccine in preventing hepatitis B virus (HBV) effect of mtct. Methods December 2014~2015 year in December 246 cases of HBsAg positive pregnant women as the research object, They were randomly divided into observation group and control group, All 123 cases. The control group only neonatal hepatitis B vaccine alone, The observation group of pregnant women and newborns were combined with hepatitis B vaccine and HBIG. Results the control group and the observation group in neonatal intrauterine HBV infection rates were 22.76%and 9.76%, A year after the control group and the observation group of neonatal HBV infection rates were 16.26%and 1.63%, The difference between the two groups were statistically significant. The positive rates of HBsAb in the control group and the observation group were 8.13%, 76.42%and 27.64%, respectively, and 94.31%, respectively, There was significant difference between the two groups. Conclusion the combined application of hepatitis B immunoglobulin and hepatitis B vaccine in pregnant women and newborns can effectively reduce the rate of neonatal HBV infection in HBsAg posi-tive cases, At the same time, it can effectively improve the HBV positive rate of the corresponding HBsAb of the newborn, and ultimately improve the transmission blocking rate.
