甲状腺微小乳头状癌超声诊断的价值及意义

目的：探讨超声在诊断甲状腺微小乳头状癌中的价值和意义。方法：选取2013年5月～2014年5月本院就诊甲状腺肿瘤患者80例，经过彩色多普勒超声诊断后，记录超声诊断出的甲状腺微小乳头状癌及其它类型肿瘤患者的例数，与临床病理结果进行对比分析。结果：经过彩色多普勒超声检查后诊断出甲状腺微小乳头状癌（d≤1cm）20例，其它类型肿瘤60例。甲状腺微小乳头状癌的彩超结果为：患者甲状腺形态不规则、边界模糊、实性不均匀回声、周围无声晕，可见微小钙化、血流紊乱。临床病理诊断结果表现是：结节边界不清晰、形态不规则、可见砂粒体，肿瘤细胞排列成乳头状结构。两种诊断方法结果比较，诊断指标相比，差异有统计学意义。甲状腺微小乳头状癌最大流速峰值与阻力指数分别为（25.89±10.56）cm/s 和（0.67±0.23），均较良性肿瘤明显增加，两者相比差异有统计学意义。结论：诊断甲状腺微小乳头状癌的重要指标是：粗糙毛刺状结节、结节内部微小钙化灶、结节形态不规则。彩色多普勒超声在诊断甲状腺微小乳头状癌中有重要意义，值得临床推广。

Value and significance of Ultrasonic diagnosis of thyroid small papillary carcinoma

Objective To discuss the value and significance of Ultrasonic diagnosis of thyroid small papillary carcinoma. Methods 80 cases of Thyroid Cancer treated in our hospitalfrom May 2013 to May 2014 were chosen.After color Doppler ul-trasound diagnosis,recorded the number of cases of thyroid small papillary carcinoma and patients with other types of tumors, Results were compared with clinical pathology results. Results Thyroid small papillary carcinoma (d≤1cm) 20 cases, 60 cases of other types of tumors diagnosed by Color Doppler ultrasound.Thyroid small papillary ultrasound results were as follows:thy-roid irregular, fuzzy boundaries, solid uneven echo, halo around silent, visible microcalcifications, blood disorders.Clinical and pathological diagnosis performance were as follows: nodule boundary was not clear, irregular, visible sand body, tumor cells were arranged in papillary structures.Comparison of two diagnostic methods results: the differences of Diagnostic indicator were statistically significant. The maximum peak velocity and the resistance index of thyroid small papillary carcinoma were (25.89 ±10.56) cm / s and( 0.67±0.23),and increased significantly higher than benign,the differences were statistically significant. Conclusion Important diagnostic indicators of thyroid small papillary carcinoma are:rough burr-like nodules, internal micro-calcifications, irregular nodules.Color Doppler ultrasound is important in the diagnosis of thyroid small papillary carcinoma and worthy of popularization.
