双切口双钢板内固定术与锁定钢板内固定术治疗复杂胫骨骨折疗效对比

目的：探讨双切口双钢板内固定术与锁定钢板内固定术治疗复杂胫骨骨折疗效对比。方法：将74例我院收治的复杂胫骨骨折患者分为两组，每组37例。研究组采用双切口双钢板内固定术治疗。对照组采用锁定钢板内固定术治疗。对比两组临床疗效及并发症发生情况。结果：研究组手术时间、出血量、开始负重时间、住院时间均显著低于对照组，差异有统计学意义；两组骨折愈合时间对比差异无统计学意义；研究组内翻角（TPA）、后倾角（PSA）显著低于对照组，膝关节活动度显著高于对照组，差异有统计学意义；研究组Rasmussen分级显著优于对照组，经秩和检验分析，差异有统计学意义；研究组关节强直、延迟愈合、膝内翻的发生率显著低于对照组，差异有统计学意义；两组膝关节不稳、不愈合、切口感染、骨坏死的发生率对比差异无统计学意义。结论：双切口双钢板内固定术治疗复杂胫骨骨折的疗效优于锁定钢板内固定术，且能显著降低并发症的发生。

Comparison of double incision double plate fixation and internal fixation with locking plate in the treatment of complex tibial fractures

Objective To evaluate the effect of double incision double plate internal fixation and locking plate fixation in the treatment of complex tibial fractures. Methods 74 cases of complex tibial fractures were divided into two groups, 37 cases in each group. The study group was treated with double incision and double plate internal fixation. The control group was treated with locking plate internal fixation. The clinical effects and complications of the two groups were compared. Results In the study group, operation time, bleeding volume, loading time and hospitalization time were significantly lower than the control group. The difference was statistically significant . The two group differences in fracture healing time was not statistically significant . The study group TPA and PSA were significantly lower than the control group, the knee joint activity was significantly higher than the control group. The difference was statistically significant . The study group was significantly better than the controlgroup. Rasmussen classification by rank test analysis, the difference was statistically significant . The study group ankylosis, delayed healing, knee varus was significantly lower than that of control group. The difference was statistically significant. Two groups of knee instability no, healing, wound infection, the incidence of osteonecrosis was compared. The difference was not statistically significant . Conclusion Double incision double plate fixation in the treatment of complex tibial fractures is better than the locking plate fixation, and can significantly reduce the incidence of complications.
