联合免疫组化和PCR检测CK20在早期宫颈鳞癌淋巴结中的表达和意义

目的：探讨早期宫颈鳞癌淋巴结微转移阳性率,阳性淋巴结分布以及与病理类型、临床分期之间的关系.方法：用免疫组化法和RT-PCR法分别及联合检测492枚（52例）早期宫颈鳞癌手术切除淋巴结中CK20的表达情况.结果：联合法检测CK20阳性率与单独使用一种方法的阳性率相比有显著性差异,且显著高于常规HE染色（P＜0.05）.闭孔淋巴结CK20阳性率最高.CK20表达与宫颈鳞癌临床分期、病理分级有关（P＜0.05）,而与年龄无关（P＞0.05）.结论：联合免疫组化和PCR检测CK20敏感性较高,闭孔淋巴结可作为检测CK20表达的首选淋巴结,对于评价早期宫颈癌微转移具有重要意义.

Expression and clinical significance of CK20 in early-stage cervical squamous cell carcinoma lympha node

Objective：To explore the positive rate of early -stage cervical squamous cell carcinoma and distribu- tion of positive lymph nodes and the relationship between the expression of CK20 and Clinical stage and Pathologic grade. Methods ： The CK20 mRNA were detected by Immunohistochemical method and RT - PCR method for 492 ca- ses of early - stage cervical squamous cell carcinoma lympha - node after the operation. Results： Combined detection for CK20 positive rate compared with single method have significant difference, and significantly higher than conven- tional HE staining（ P 〈0.05 ）. The highest positive rate was obturator lymph nodes. The obvious correlations were obtained between the expression of CK20 mRNA and the clinical stage and pathologic grade（ P 〈 0.05 ） ,but the sig- nificant relations were not found between the expression of CK20 mRNA and age （ P 〉 0.05 ）. Conclusion ： Combined immunohistochemistry with PCR detection of CK20 has higher sensitivity obturator lymph nodes can be used as detecting lymph nodes for CK20 expression, to evaluate the early cervical cancer metastasis is of great significance.
