肾上腺素与血管加压素对窒息性心脏骤停大鼠早期心肺复苏的影响

摘    要:目的:比较肾上腺素与血管加压素对室息性心脏骤停大鼠早期心肺复苏的影响。方法:健康SI)夫鼠48只,体重200~250g,雌雄产拘,将大鼠随机分成4组(n=12):正常对照组(C组)、模型组(M组)、肾上腺素组(E组)和血管加雎素组(V组)。M组、E组及V绀大鼠均经气管夹闭1min,建立窒息性心脏骤停模型。肖窒息时间达1min时,分别在常规心肺复苏前:c组和M组大鼠经股静脉注射生理盐水1m1。E组及V纰大鼠经股静脉分别注射肾上腺素0.04mg/kg及血管加压素0.4u/kg,H时开始胸外心脏按压及机械通气,观察自主循环恢复情况,5min无效则放弃复苏。自主循环恢复的大鼠连续监测心电和血压30min。记录在CPR30min内自主循环恢复情况及血流动力学;记录CPR30rain后,采用免疫组织化学法检测各组实验大鼠血清中肿瘤坏死因子-水平、白细胞介素6及白细胞介素10水平。结果:E、V组大鼠自主循环恢复率均显著高于M组(分别为73.4%.74.6%,和15.3%,P〈0.05).E组大鼠自主循环恢复牢与V组比较差异无统计学意义(P〉0.05);E、V组大鼠平均动脉压明显高于M绀大鼠(P〈0.05),V组大鼠MAP高于E组大鼠。差异有统计学意义(P〈0.05)。M组大鼠血清中自细胞介素6及肿瘤坏死因子-水平与E、V组差异有统计学意义(P〈0.05);E组大鼠m清中肿瘤坏死阔子-及白细胞介素-6水平高于V组(P〈0.05)。E组大鼠血清中白细胞介素-10水平低于V组(P〈0.05)。结论:肾卜腺素与血管加压素在窒息性心脏骤停大鼠早期心肺复苏过程中复苏成功牢无明显差异,但血管加乐素町维持大鼠复苏后平均动脉搓在相对较高水平;同时血管加压素可提高复苏后大鼠血清中抗炎因子水平。

Effect of epinephrine and vasopressin during early cardiopulmonary resuscitation in a mouse model of asphyxial cardiac arrest

Abstract:Objective:The purpose of this study was to compare the effects of epinephrine and vasopressin dur- ing early cardiopulmonary resuscitation (CPR) in a mouse model of asphyxia] cardiac arrcsl. Method: Forty-eight SD rats of both sexes weighing 200~250 g were randomized into 4 groups(n- 12 each) and receive saline (C,M- gro, 1 ml i. v) ,epinephrine group(E gro,0.04 mg/kg i. v) ,vasopressin group (V-gro,0.4 u/kg i. v) ,then CPR was initiated. Restoration of spontaneous circulation (ROSC) was observed. When there was a failure to restore sponta- neous circulation after 5 rain, resuscitalion efforts were discontinued. Electrocardiogram and bloocl pressure of re- suscitated animals were inwlsively monitored for an additional 30 minutes. R()S(? and hemodynamics were observed and immediately recorded. Immunohisto chemistry technique was used to detect interleukin-6, interleukin-10 and tumor necrosis factor-levels. Result:Within 30 minutes of restoration of spontaneous circulation, rates of R()SC in E-gro and V gro were significantly higher than in M-gro (73. 4% ,74.6% ,和 15.30% ,P〈0.05) ,here was no ob- vious difference between E-gro and V gro(P5〉0.05). MAP in E gro and V-gro were significantly higher than that in M-gro(P〈0. 05). MAP in V-gro was higher than in E-gr0(P〈20.05). The levels of inter|eukin-6 and tumor necrosis facto〉were higher in E-gro than those in V-gro(P〈0.05);The level of interleukin 10 was higher in V- gro than that in E-gro(P〈0.05). Conclusion: The success rates of vasopressin and epinephrine are similar in early cardiopulmonary resuscitation,but wssopressin can maintain a high-level mean arterial pressure after resuscitation, and increase the level of anti-inflammatory factor.
