CTnT、Mb、NT—proBNP、hs—CRP联合检测在急性心肌梗死诊断中的价值分析

目的探讨血CTnT、Mb、NT-proBNP、hs—CRP联合检测在诊断急性心肌梗死的临床应用价值。方法测定102例稳定性心绞痛患者（SAP组），106例不稳定性心绞痛患者（UAP组），120例急性心肌梗死（AMI）患者和105例健康人血中心肌肌钙蛋白T（cTnT）、肌红蛋白（Mb）、N末端前B型脑钠肽原（NT—proBNP）、超敏c反应蛋白（hs—cap）含量，各组之间相互比较。结果急性心肌梗死患者血中CTnT、Mb、NT—proBNP、hs-CRP水平显著高于SAP组，UAP组和对照组，差异有显著统计学意义（p〈0．01），CTnT特异性最高，Mb灵敏度最高，特异性较差。结论cTnT、Mb、NT—proBNP、hs—CRP对AMI诊断有重要临床意义，联合检测可以提高AMI诊断的灵敏度和特异性，具有较高临床价值。

THE SIGNIFICANCE OF COMBINED CTnT,Mb,NT-proBNP AND hs-CRP DETERMINATION IN DIAGNOSING ACUTE MYOCARDIAL INFARCTION

Objective To evaluate the significance of combined CTnT, Mb, NT - proBNP and hs - CRP determination in diagnosing acute myocardial infarction. Methods Serum samples of 102 stable angina peetoris （SAP） patients, 106 unstable angina pectoris （UAP） patients, 120 acute myocardial infarction （AMI） patients and 105 eontrel group were recruited in this study. The levels of CTnT, Mb, NT - proBNP and hs - CRP were measured simul- taneously and different groups were compared. Results The levels of CTnT, Mb, NT - proBNP and hs - CRP were significantly higher in AMI group than other three groups （p 〈 0. 01 ）. The highest specificity is in CTnT, the higher sensitivity in Mb, but less specificity. Conclusion The levels of CTnT,Mb, NT - proBNP and hs - CRP have very high diagnosis value in diagnosing acute myocardial infarction. Joint detection can improve the sensitivity and speeifieity of AMI diagnosis with high clinical value.
