三维适形放疗治疗食管癌术后食管气管沟淋巴结转移的疗效

目的 探讨三维适形放疗（3DCRT）治疗食管癌术后食管气管沟淋巴结转移的疗效及影响预后的因素.方法 46例食管癌术后食管气管沟淋巴结转移患者接受3DCRT.结果总有效率为87.0%,中位生存期15个月,1、2、3 a生存率分别为63.0%、30.4%、2.2%.生存分析显示患者性别、年龄、术前肿瘤部位、病变长度、病理分型、术后分期、术后食管气管沟淋巴结转移部位、转移淋巴结长径对预后有显著影响（P〈0.05）.主要毒副反应为放射性食管炎和放射性肺炎,Ⅰ、Ⅱ级放射性食管炎发生率65.2%（30/46）,Ⅰ、Ⅱ级放射性肺炎发生率45.7%（21/46）.结论 3DCRT治疗食管癌术后食管气管沟淋巴结转移安全有效.

Effect of Three-dimensional Conformal Radiotherapy in the Treatment of Tracheoesophageal Groove Lymph Node Metastasis from Esophageal Carcinoma After Surgery

Objective To evaluate the effect of three-dimensional conformal radiotherapy （3DCRT） in the treatment of tracheoesophageal groove lymph node metastasis from esophageal carcinoma after surgery. Methods Forty- six patients with tracheoesophageal groove lymph node metastasis from esophageal carcinoma after surgery were treated by 3DCRT. Results The local control rate was 87.0%. The median survival time was 15 months. The 1-, 2-, 3-year survival rates were 63.0% , 30.4% , 2.2% , respectively. The gender, age, the site of tumor, the length of treMor, pathological elassification, clinical stage, the location and length of lymph node metastasis affected the prog- nosis （P 〈0.05 ）. The main radiation related toxicities were acute radiation esophagitis and acute radiation pneumonitis. The incidence of acute radiation esophagitis with grade I and II was 65.2% （30/46）, that of the acute radiation pneumonitis with grade I and II was 45.7% （21/46）. Conclusion 3DCRT is safe and effective for tracheoesophageal groove lymph node metastasis from esophageal carcinoma after surgery.
