胸部手术后肺部并发症的预防护理效果研究

目的 分析比较传统拍背咳痰与振动排痰机对胸部手术后肺部并发症的预防护理效果.方法 选择60例胸部手术患者,随机分为实验组与对照组各30例.实验组使用振动排痰机辅助排痰,对照组使用传统拍背咳痰.两组预防措施实施相同时间后对患者胸部术后发生低氧血症、肺不张、需纤维支气管镜介入吸痰情况,以及术后6 h与术后48h的动脉血气结果 进行对比分析,同时观察两组病例的排痰感受.结果 两组患者发生低氧血症、肺不张、需纤支镜介入吸痰情况,实验组均明显少于对照组;实验组感觉舒适与对照组相比有显著性差异;实验组疼痛、恶心等感受与对照组相比较,均有显著性差异;术后48 h的动脉氧分压(PaO2)、动脉血二氧化碳分压(PaCO2)与术后6 h比较均有显著性差异,pH值两者前后无统计学意义,但实验组术后48 h与术后6 h的PaO2、PaCO2差值与对照组术后48 h与术后6 h的差值相比较,差异具有显著性.结论 使用振动排痰机对开胸术后肺部并发症的预防与治疗效果显著.

The preventive nursing effect on pulmonary complications for patients after thoracic operation

Objective To compare the preventive effect,the sputum excretion effect and the amelioration of respiratory function of vibratory sputum ejection apparatus(VIBSEA) and those with traditional sputum ejection measures on pulmonary complications for patients underwent thoracic operation.Methods 60 patients underwent thoracic operation were selected and randomly divided into control group and experiment group.The occurrence of hypoxemia and pulmonary atelectasis and sputum ejection by fiberoptic bronchoscopy were compared.The arterial blood gas concentration was measured at 6 hours and 48 hours after the operation,and the feeling of sputum excretion was also observed.Results The occurrence of hypoxemia,pulmonary atelectasis and sputum ejection by fiberoptic bronchoscopy in patients of experiment group was significantly less than those in control group.The cases with comfortable feeling in control group were less than that in experiment group,but the cases with painful feeling in control group were more than that in experiment group.The differences in PaO2 and PaCO2 at 48 hours and 6 hours after operation were significant.Conclusion The use of VIBSEA is safe,well-tolerated,comfortable and effective in preventing pulmonary complications in patients undergone thoracic operation.
