心脏直视手术中自体血回输的临床应用研究

目的探讨在心脏直视手术中应用自体血回输的临床效果。方法将98例心脏直视手术患者随机分为两组,观察组采取预存式和血液等容稀释式自体血输血相结合的方法,对照组未输自体血,比较两组患者手术前后的Hb、PLT、术中失血量、术后引流量、用异体血量及术后并发症等情况。结果两组患者术前及术后24小时的Hb差异无显著性（P〉0.05）;输库血量、术后引流量对照组明显多于观察组,差别有高度显著性（P〈0.01）;术后24小时PLT对照组较观察组明显减少,差异有显著性（P〈0.05）;术后并发症对照组明显高于观察组,差异有高度显著性（P〈0.01）。结论心内直视手术患者采用自体血回输,安全可靠,可明显减少输异体血量、术后引流量及术后并发症的发生率。

Study on autogenous blood transfusion in open heart surgery

Objective: To study the clinical effects of autogenous blood transfusion in open heart surgery.Methods: 98 patients were randomly divided into two groups.In the observed group,the synthetic autogenous blood was transfused.In the control group,the banked blood was given only.The value of Hb and PLT of pre and postoperative in two groups were contrasted.The volume of drainage,intraoperative losing blood and banked blood use were recorded.The complications in two groups were also recorded.Results: comparing with two groups,the Hb and the intraoperative losing blood weren't significant deference(P>0.05).But in the postoperative drainage volume and transfused heterogeous blood,the control group was more than that observed group(P<0.01).The PLT of postoperation 24 hours in the observed group decreased than the control group(P<0.05).The operative complications which control group was higher than that of observed group(P<0.01).Conclusion: Autogenous blood transfusion is suitable for open heart surgery.The heterogeous blood transfusion,postoperative drainage volume and operative complications can be reduced obviously.
