不同类型上皮性卵巢癌中卵巢与输卵管病变累及关系的研究

目的:研究不同病理类型上皮性卵巢癌的输卵管累及情况,探讨浆液性卵巢癌起源于输卵管上皮的可能性。方法:回顾分析2008年10月至2013年2月在同济大学附属第一妇婴保健院进行初次手术治疗的146例上皮性卵巢癌患者的临床病理资料,比较不同病理类型对输卵管的累及,并比较高级别浆液性癌(HGSC)和低级别浆液性癌(LGSC)的临床病理资料。结果:上皮性卵巢癌中,浆液性癌有69.9%累及输卵管,高于其余病理类型;输卵管累及的病例中,卵巢的病理类型为浆液性癌者占90.6%。浆液性癌中,HGSC组的晚期病例(FIGOⅢ、Ⅳ期)数、累及双侧卵巢、累及输卵管、累及腹膜及大网膜的情况均高于LGSC组,差异有统计学意义(P0.05)。结论:高级别浆液性卵巢癌较其他病理类型,同时发生输卵管病变的情况更多见,提示输卵管可能为浆液性卵巢癌的起源之一。

The study on the relationship between involvement of ovary and fallopian tube in epithe-lial ovarian cancer

Objective：To study the fallopian tube involvement of different pathological type of epithelial ovarian cancer, and to discuss the possible origin of serous ovarian cancer. Methods：A total of 146 cases undergoing operative treatment in Shanghai First Maternity and Infant Hospital,Tongji University from Oct. 2008 to Feb. 2013 were enrolled in this respective study. The fallopian tube involvement of different pathological type of epithelial ovarian cancer were observed,and the clinicopathologic data of high grade serous carcinoma（ HGSC） and low grade serous carcinoma（LGSC） were compared. Results：69. 9% of serous carcinoma involved fallopian tube,higher than other pathological type. Among the fallopian tube involvement cases, 90. 6% were serous carcinoma. Late stage cases,bilateral ovary involvement,fallopian tube in-volvement,peritoneum and omentum involvement were statistically higher in HGSC than in LG-SC. Conclusion：Fallopian tube involvement is more common in HGSC, compared with other pathological type of epithelial ovarian cancer. Fallopian tube is one of the possible origins of se-rous serous ovarian cancer.
