小剂量强的松治疗慢性阻塞性肺部疾病急性加重的疗效及疗程研究

目的:了解小剂量强的松(20mg/d)治疗慢性阻塞性肺部疾病急性加重(AECOPD)是否有效,口服小剂量强的松5d和14d对AECOPD的疗效是否相同。方法:对180例AECOPD患者随机分成3组:5d治疗组、14d治疗组和安慰剂组。治疗前、后观察呼吸困难改善程度、肺功能FEV1预计值改善程度、血气分析PaO2升高程度及住院天数、治疗失败率、激素副作用、复发率等。结果:治疗组与安慰剂组比较,呼吸困难改善程度及肺功能提高明显,而5d治疗组与14d治疗组比较,差异不明显。结论:口服小剂量强的松治疗AECOPD有效,且5d疗程与14d疗程无明显差异,可明显缩短疗程,并减少激素副作用。

Effect of Small Dose of Prednisone in the Treatment of AECOPD and Course of Study

Objective： The understanding of small dose prednisone （20mg/d）in the treatment of AECOPD is effective, the efficacy of oral low-dose prednisone 5d and 14d of AECOPD are the same. Methods： In 180 cases of AECOPD patients were randomly divided into 3 groups：5d treatment group, 14d treatment group and placebo group. Before and after treatment, dyspnea, pulmonary function solution good degree of FEV1 is expected value solution good degree, blood gas analysis of elevated PaO2 level and days of hospitalization,treatment failure rate, the side effect of hormone, recurrence rate. Results： The treatment group compared with the placebo group, respiratory difficult solution good degree and the pulmonary function was improved, and the 5d treatment group and 14d treatment group, no significant difference. Conclusion： Small doses of oral prednisone treatment AECOPD effective, and 5d treatment and 14d treatment have no obvious difference, can obviously shorten the period of treatment, and reduce the side effects of hormone.
