心肌肌钙蛋白T(cTnT)在心肌炎中的动态变化及与心功能的相关性

目的:探讨心肌肌钙蛋白T(cTnT)在心肌炎中的动态变化及其与心功能的相关性。方法:对2007年7月～2012年7月入住我院的28例心肌炎(心肌炎组)、24例疑似心肌炎(疑似心肌炎组)以及32例健康体检者(健康对照组),检测各组cTnT、肌酸激酶(CK)、肌酸激酶同工酶(CK-MB)水平,且对cTnT与心功能相关性进行分析。结果:(1)心肌炎组cTnT浓度明显高于疑似心肌炎组与健康对照组[(0.37±0.13)ng/ml比(0.19±0.07)ng/ml、(0.17±0.05)ng/ml),P<0.01];(2)cTnT与心脏指数(CI)、左室射血分数(LVEF)、肺动脉血流加速时间(AT)、相对室壁厚度(RWT)均呈负相关性(r=-0.723～-0.441,P<0.05或<0.01),与左室质量指数(LVMI)、左室舒张末期内径(LVEDd)、左室内径(LAD)、室间隔厚度(IVST)及左室后壁厚度(LVPWT)均呈正相关性(r=0.387～0.763,P均<0.05)。结论:心肌肌钙蛋白T水平是评价心肌炎心功能的一个重要指标。

Dynamic changes of cardiac troponin T in myocarditis and its correlation with cardiac function

LI Xiao-li//Department of Pediatrics,Central Hospital of Ankang City,Ankang,Shaanxi,725000,China Objective：To explore dynamic changes of cardiac troponin T（cTnT）in myocarditis and its correlation with cardiac function.Methods：A total of 28patients with myocarditis（myocarditis group）,24patients suspected myocarditis（suspected myocarditis group）and 32healthy subjects（healthy control group）admitted in our hospital from Jul 2007to Jul 2012were enrolled.Levels of cTnT,creatine kinase（CK）and CK-MB were measured in three groups,and correlation among cTnT and indexes of cardiac function were analyzed simultaneously.Results：（1） Compared with suspected myocarditis group and healthy control group,there was significant increase in cTnT concentration[（0.19±0.07）ng/ml,（0.17±0.05）ng/ml vs.（0.37±0.13）ng/ml]in myocarditis group,P〈0.01;（2）cTnT was negatively correlated with cardiac index（CI）,left ventricular ejection fraction（LVEF）,pulmonary artery blood flow acceleration time（AT）and relative wall thickness（RWT）,r=-0.723～-0.441,P〈0.05or 0.01;and cTnT was positively correlated with left ventricular mass index（LVMI）,left ventricular end-diastolic dimension（LVEDd）,left atrial diameter（LAD）,interventricular septal thickness（IVST）and left ventricular posterior wall thickness（LVPWT）,r=0.387～0.763,P〈0.05all.Conclusion：Cardiac troponin T level is an important index evaluating cardiac function in patients with myocarditis.
