老年急性心肌梗死患者肌红蛋白,肌钙蛋白T、肌酸激酶MB同工酶的检测意义

摘    要:目的探讨血清肌红蛋白(Mb)、肌钙蛋白T(cTnT)和肌酸激酶同工酶(CK-MB)对老年急性心肌梗死溶栓疗效早期的检测意义。方法应用酶联免疫分析法测定32例老年急性心肌梗死患者溶栓治疗后的测定Mb、cT-nT、CK-MB浓度,分析老年急性心肌梗死患者溶栓再通组(n=18)和溶栓未通组(n=14)上述指标的变化情况。结果急性心肌梗死溶栓再通组Mb、cTnT和CK-MB达到峰值浓度的时间明显较未通组提前(P<0.05),其中,Mb较cTnT和CK-MB峰值出现更早,分别为(7.4±2.5)h,(13.7±4.1)h和(14.4±2.7)h(P<0.01);Mb的诊断敏感性(79%)与诊断效率(87%)明显高于cTnT(60%、75%)和CK-MB(47%、67%)(P<0.01)。结论血清Mb、cTnT和CK-MB水平和变化情况可以较好地早期预测老年急性心肌梗死患者溶栓再通,其中Mb较cT-nT及CK-MB具有更好的临床检测意义。

Abstract:Objective:To investigate and compare the clinical usefulness of serum myoglobin(Mb),troponin T(cTnT), and creatine kinase MB(CK-MB) mass in the early detection of reperfusion after thrombolytic treatment in aging. Methods: Using the method of ELISA, serial blood samples were taken from 32 patients with acute myocardial infarction. Concentrations of Mb, cTnT, and CK-MB were assayed in each sample. Thrombolytic treatment was given to the patients who were divided into those who reperfused(n=18) and those who failed to reperfuse(n=14).Results:The time to reach peak value of Mb, cTnT,and CK-MB were earlier in the patients who reperfused than those of patients who unreperfused, and Mb is more early than cTnT and CK-MB; The sensitivity and diagnosis efficiency of Mb were higher than those of cTnT and CK-MB.Conclusion:The results suggest that Mb has greater potential than cTnT and CK-MB examined in the detection of reperfusion after thrombolytic treatment in aging.
