摘    要:目的探讨3.0TMR扩散加权成像(DWI)及相应的表观扩散系数(ADC)对胰腺癌的诊断价值。方法对13例志愿者及13例胰腺癌患者在3.0TMR上行胰腺DWI成像。比较DWI与T1WI、T2WI序列中胰腺癌相对于癌周的信号强度比(SIR);比较胰腺癌、癌周及志愿者正常胰腺的ADC值并绘制ADC值诊断胰腺癌的ROC曲线。结果DWI及T1WI图像中胰腺癌相对于癌周的SIR均明显高于T2WI(P<0.05),而DWI与T1WI相比则没有显著差异(P>0.05)。胰腺癌与癌周及志愿者正常胰腺的ADC值均有显著性差异(P<0.05)。ROC曲线下的面积为(0.739±0.088)(P<0.05)。结论DWI成像及ADC值的测定可能会在一定程度上协助胰腺癌的诊断。

Abstract:Objective To determine the potential ability of diffusion-weighted imaging (DWI) and corresponding apparent diffusion coefficient (ADC) in the diagnosis of pancreatic cancer at 3.0T MR. Methods DWI of pancreas was performed in 13 patients suffered from pancreatic cancer and 13 volunteers with normal pancreas. The pancreatic cancer-pericancerous tissue signal intensity ratio (SIR) in DWI, T1WI and T2WI were compared. The ADC values of the cancer, pericancerous tissue and normal pancreas of volunteers were statistically analyzed. The ROC curve of ADC value in diagnosis of pancreatic cancer was obtained. Results The pancreatic cancer-pericancerous tissue SIR in DWI and T1WI were higher than that in T2WI (P<0.05), but there was no difference between DWI and T1WI (P>0.05). The ADC value of pancreatic cancer was significantly different from that of the pericancerous tissue and normal pancreas of volunteers (P<0.05). The area under the ROC curve was (0.739±0.088) (P<0.05). Conclusion DWI may do some contribution to the diagnosis of pancreatic cancer.
