血浆B型钠尿肽在冠心病严重程度及预后评估中的价值

目的探讨血浆BNP水平与CHD严重程度和预后的关系。方法比较250例CHD患者和40例对照组血浆BNP水平,分析不同病变程度CHD患者血浆BNP值变化及相关性。结果对照组与CHD组血浆BNP值之间有显著差异（P〈0.01）,SAP、UAP、AMI组血浆BNP水平、Gensini评分之间有显著性差异（P〈0.01）,血浆BNP水平与Gensini评分、NYHA分级呈正相关（P〈0.01）。血浆BNP值为204pg/mL时,预测CHD不良心脏事件敏感度和特异性较高。低危组和高危组不良心脏事件发生率和死亡率之间有显著性差异（P〈0.01）。结论 CHD患者血浆BNP显著升高,血浆BNP水平对CHD患者病变严重程度及预后评估具有重要的临床意义。

Value of plasma B-type natriuretic peptide for severity and prognosis of coronary heart disease

Objective To investigate the value of plasma B-type natriuretic peptide(BNP) for severity and prognosis of coronary heart disease(CHD).Methods Compared plasma BNP levels of CHD group and control group.Analyzed plasma BNP levels changes and relativity of different severity in patients with CHD.Results There were significant difference of the plasma BNP levels between CHD and control group(P <0.01).When the plasma BNP levels was 204pg/mL,there were higher sensitivity and specificity of assessing adverse cardiac events.There were significant difference of the adverse cardiac events rate and death rate between low risk and high risk group(P < 0.01).Conclusion The plasma BNP levels of CHD is significantly increased,and a useful marker for the severity and prognosis of CHD.
