D-二聚体、凝血指标、血小板参数检测对妊娠高血压疾病的应用价值分析

目的：观察和分析 D-二聚体（D-D）、凝血指标、血小板参数检测对妊娠高血压疾病（HDCP）的应用价值。方法选取60例 HDCP 患者作为 HDCP 组，选取足月妊娠待产健康孕妇60例作为妊晚组，选取健康育龄非孕妇女60例作为对照组，对三组研究对象的 D-D 水平、凝血酶原时间（PT）、活化部分凝血活酶时间（APTT）、凝血酶凝结时间（TT）、纤维蛋白原（FIB）、血小板计数（PLT）、血小板压积（PCT）、平均血小板体积（MPV）、血小板体积分布宽度（PDW）等指标进行检测和比较。结果HDCP 组的 D-D 水平显著高于妊晚组（P ＜0．05），妊晚组显著高于对照组（P＜0．05）；对照组的 PT 水平显著高于妊晚组（P ＜0．05），妊晚组显著高于 HDCP 组（P ＜0．05）；妊晚组的 APTT 水平显著低于 HDCP 组和对照组（P 均小于0．05）；HDCP 组的 FIB 水平显著高于妊晚组（P ＜0．05），妊晚组显著高于对照组（P ＜0．05）；对照组的 PLT 水平和 PCT 水平均显著高于 HDCP 组和妊晚组（P 均小于0．05），HDCP 组的 MPV 水平和 PDW 水平均显著高于妊晚组和对照组（P 均小于0．05）；随着 HDCP 患者病情程度的加重，其 D-D 水平呈现显著升高的趋势（P 均小于0．05）。结论HDCP 患者机体的高凝状态和继发性纤溶状态均显著高于正常孕妇和非妊娠妇女，血栓形成的趋势和危险性较高，且 D-D 水平与患者的病情可能存在正相关关系，联合应用 D-D 水平、凝血指标、血小板参数检测对患者进行监测对于 HDCP 的早期诊断和及时治疗具有重要的意义。

The analysis on the value of the application of D-dimer,coagulation parameters,platelet parameters detection to the patients with hypertensive disorder complicating pregnancy

Objective To observe and analyze the value of the application of D-dimer （D-D）,coagulation parame-ters,platelet parameters detection to the patients with hypertensive disorder complicating pregnancy （HDCP）.Methods 60 patients with HDCP were selected as the HDCP group,60 cases of healthy women with full-term pregnancy were selected as the late pregnancy group and 60 cases of healthy non pregnant women in childbearing age were selected as the control group.The D-D levels,prothrombin time （PT）,activated partial thromboplastin time （APTT）,thrombin condensation time （TT）,fibrinogen （FIB）,platelet count （PLT）,platelet hematocrit （PCT）,mean platelet volume （MPV）,platelet volume distribution width （PDW）of the subjects in the three groups were tested and compared.Re-sults The D-D level of the HDCP group was significantly higher than that of the late pregnancy group （P 〈0.05）and the level of the late pregnancy group was significantly higher than that of the control group （P 〈0.05）;The PT level of the control group was significantly higher than that of the late pregnancy group （P 〈0.05）and the level of the late pregnancy group was significantly higher than that of the HDCP group （P 〈0.05）;The APTT level of the late preg-nancy group was significantly lower than that of the HDCP group or the control group （Ps were all less than 0.05）;The FIB level of the HDCP group was significantly higher that of the late pregnancy group （P 〈0.05）and the level of the late pregnancy group was significantly higher than that of the control group （P 〈0.05）;The PLT level and PCT level of the control group were significantly higher than those of the HDCP group or the late pregnancy group （Ps were all less than 0.05）,while the MPV level and PDW level of the HDCP group were significantly higher than those of the late pregnancy group or the control group （P 〈0.05）;With the aggravation of HDCP,the D-D levels of the patients in-creased significantly （P 〈0.05）.Conclusion The hypercoagulability and the secondary fibrinolysis of the patients with HDCP are more significant than those of the normal pregnant women and non pregnant women.The trend and the risk of thrombosis are higher.Furthermore,there may be a positive correlation between the D-D level and the Severity of the patients’symptoms.The combined application of D-D level,coagulation parameters,platelet parameters detection to＆amp;nbsp;monitor the patients with HDCP is of great significance for early diagnosis and timely treatment.
