血清脂联素在新生儿缺血缺氧性脑病中的诊疗价值

摘    要:目的 探讨血清脂联素是否可以辅助用于新生儿缺血缺氧性脑病的诊疗,以及疾病严重程度的判断.方法 分析2011年1月-2013年1月在衢州市妇幼保健院接受治疗的新生儿缺血缺氧性脑病(HIE)患儿的临床资料,另选取25例健康新生儿作为对照组.结果 共纳入HIE患儿30例.对照组平均血清脂联素水平显著高于HIE患儿(P<0.05).HIE患儿随着疾病严重级别的上升,血清脂联素呈现下降趋势.轻度、中度及重度HIE患儿治疗4周后血清脂联素较治疗前均显著升高(P<0.05).HIE患儿治疗4周后血IL-6水平显著低于治疗前(t=9.317,P<0.001);HIE患儿治疗4周后血CRP水平显著低于治疗前(t=11.450,P<0.001).HIE患儿血IL-6水平与血清脂联素呈现显著负相关(r=-0.688;P<0.001).HIE患儿血CRP水平与血清脂联素呈现显著负相关(r=-0.575;P<0.001).结论 血清脂联素随着HIE的严重度增高而升高,其与CRP、IL-6具有负相关性,综合这些指标可用以评估HIE患几的预后与疗效.

Abstract:Objective Our prospective study was aimed to analyze the value of adiponeetin in the diagnosis of hypoxie-is- chemic eneephalopathy. Methods Clinical data of patients with hypoxie-isehemie eneephalopathy(HIE) received treat- ment at our hospital from 2011 to 2013 was prospective analyzed and selected 25 healthy newborn as control group. Re- suits A total of 30 HIE patients were prospective analyzed. HIE patients had a lower level of adiponectin than that of control group and when the disease severity classification was higher,the level of adiponectin was lower. HIE patients got a higher level of adiponectin after treatment than before, the difference was statistically significant (P 〈 0.05 ). After treat- ment, HIE patients got a lower level of CRP than before, the difference was statistically significant ( t = 11. 450, P 〈 0. 001 ). While HIE patients also got a lower level of IL-6 than before ( t = 11. 450,P 〈 0. 001 ). We found that IL-6 had a significant negative correlation with adiponeetin( r = -0.688 ;P 〈 0.001 ) and CRP also had a significant negative corre- lation with adiponeetin ( t = 11. 450, P 〈 0.001 ). Conclusion Adiponectin combine with IL-6, CRP can be used in diag- nosis and judgment prognosis of hypoxie-isehemie eneephalopathy.
