经颅多普勒超声对桥脑小脑角区肿瘤供血动脉“盗血”及术后血流情况的观察

目的探讨经颅多普勒超声（TCD）在观察桥脑小脑角区肿瘤供血动脉血流动力学变化中的价值。方法应用TCD检查24例伴“盗血”现象的桥脑小脑角区肿瘤患者的主要供血动脉以及相关动脉，于术前、术后进行血流方向、血流速度、搏动指数（PI）、阻力指数（RI）和频谱形态等血流动力学检查，分析比较各项参数在手术前后的变化。结果桥脑小脑角区主要供血动脉和相关动脉的血流方向、血流速度、PI、RI及频谱形态在手术前后有显著变化：血流方向由反向恢复顺向，术后各动脉的平均血流速度（Vm）较术前明显变缓，PI、RI降低，由术前紊流频谱恢复至层流频谱。结论TCD可观察桥脑小脑角区肿瘤的盗血现象，并对手术疗效进行间接评估。

Observation on artery stealing blood of eerebellopontine angle tumor and hemodynamic after operation by transrranial Doppler ultrasound

Objective To explore the value of transcranial Doppler ultrasound （TCD） in observing hemodynamic change of cerebellopontine angle tumor. Methods Twenty -four patients with eerebellopontine angle tumor and ＂stealing blood＂ were enrolled in this study. Their main feeding artery and correlated artery were detected by TCD, and the hemodynamie parameters, such as blood direction, blood velocity （Vm） , pulsatility index （PI） , resistance index （RI） and spectrum character were examined before and after operation, then the changes of each parameter were analyzed. Results There was significant difference in blood direction, blood velocity, PI, RI and spectrum character between main feeding artery and correlated artery. The artery blood flows reverted to normal direction after operation, and the mean blood flow velocity was slower than that before operation, PI and RI were decreased, The circumscribed flow disturbances （turbulence and musical murmur） abated after the operation. Conclusion TCD can observe steal phenomenon of cerebellopontine angle tumor effectively, and evaluate the therapeutic effect after operation indirectly.
