摘    要:目的:探讨临床上各种海马杏仁核切除手术入路的优劣。方法:依据手术特点对15例(30侧)成人尸体头颅颞叶进行巨微解剖。结果:脉络膜裂是重要解剖间隙,其内有海马动态、脉络膜前动脉走行,海马动脉是海马结构的主要供血动脉;大脑后动脉走行于环池内,与海马旁回间有蛛网膜相隔。结论:现有的经颞叶外侧皮层、经侧层、经颞底等入路各有其优劣,而经颧弓颞叶入路是相对安全的入路。

Abstract:Objective To study the advantages and disadvantages of various selective amygdalo-hippocampectomy approaches. Methods The macro-micro-anatomy of temporal lobe was studied in 15 adult cadaver brains. Results Arachnoid fissure is an important surgical structure. Hippocampus artery and anterior arachnoid artery pass through this structure. Hippocampus artery is the main supplying artery to hippocampus. Posterior cerebral artery lies in the ambient cistern, it is isolated from the parahippocampus gyrus by arachnoid member which should be preserved during operation. Conclusion Transcortical, transsylvian, transtemporal-base approachos have their advantages and disadvantages. Transzygmatic-temporal-base approach is a relatively safe approach.
