检测无重合护理干预对慢性乙型肝炎患者行阿德福韦酯抗病毒治疗疗效的影响

目的研究护理干预对慢性乙型肝炎（CHB）患者行阿德福韦酯抗病毒治疗疗效的影响。方法选取2009年8月—2012年8月在本院肝病科住院,经治疗病情好转后予以出院,但出院后需进一步行阿德福韦酯抗病毒治疗的的慢性乙型肝炎患者80例,随机分成对照组和干预组各40例。对照组进行常规的用药宣教,干预组在对照的基础上进行全程、个体化、专业的护理干预,1年后测定两组患者的肝功能、HBV水平,并调查患者对医学常识的掌握水平、抑郁、睡眠、遵医行为、治疗信心等情况。结果抗病毒治疗1年后,护理干预组的医学常识缺失率、抑郁发生率、失眠发生率均较对照组显著下降（均P〈0.05）,而遵医行为和治疗信心均较对照组显著提高（均P〈0.05）;护理干预组的ALT、AST、HBV-DNA水平均显著低于对照组（均P〈0.05）,而HBsAg、T-biL水平两组间无显著差异（均P〉0.05）。结论全程、个体化及专业的护理干预可显著提高CHB患者行阿德福韦酯抗病毒治疗的疗效。

The contribution of intensive nursing care to the treatment of chronic hepatitis B with Adefovir Dipivoxil

Objective The study was intended to learn about the contribution of intensive nursing care to the treatment of chronic hepatitis B （CHB） Adefovir Dipivoxil. Method Eighty hospitalized CHB patients were randomly selected and divided into an experimental group and a control group. The control group were given normal nursing care, while the experimental group were given intensive nursing care. The levels of ALT, AST, HBsAg and HBV-DNA obtained one year after intensive nursing were compared with those obtained before intensive nursing was exercised. Findings There was no obvious difference of gender, age and degree of education between both groups （ P 〉 0.05 ）. There was no significantdifference the levels of ALT, AST and HBV-DNA between the two groups before intervention（P 〉 0.05）. After one year follow-up management, compared with the control group, the levels of ALT, AST and HBV-DNA of patients in the control group decreased significantly（ P 〈 0.05 ）. The loss rate of HBV-DNA significantly increased（ P 〈 0.05 ）. Conclusion The follow-up management can improve the liver function of chronic hepatitis B patients treated with Adefovir Dipivoxil therapy and promote the loss rate of HBV-DNA.
