摘    要:【目的】探讨颅内动脉狭窄的相关危险因素。【方法】对320例急性缺血性脑卒中患者进行颅内血管磁共振血管成像(MRA)联合经颅多普勒超声(TCD)或脑血管数字减影血管造影(DsA)检查,其中270例(84.4%)颅内动脉存在不同程度狭窄患者作为病例组,50例(15.6%)无明显颅内动脉狭窄患者作为对照组,比较两组患者的临床特征。【结果】单变量分析发现两组患者间高血压、脂代谢紊乱、缺血性心脏病发生率差异均有显著性(P〈0.05);多元logistic回归分析发现,高血压、糖尿病、脂代谢紊乱是颅内动脉狭窄的独立危险因素。【结论】高血压、糖尿病、脂代谢紊乱可能是导致颅内动脉狭窄形成的独立危险因素,应进行大样本的队列研究加以证实。

Abstract:[Objective] To investigate the correlation factors of intracraniat artery stenosis. [Methods] A total of 320 acute ischemic stroke patients were included in the study. They were all completed the cerebral vascular examinations by magnetic resonance angiography(MRA) combined with transcranial Doppler sonography(TCD) or digital subtraction angiography(DSA). Among them, 270 cases(84.4 %) with various intracranial artery stenosis were selected as case group and 50 cases(15.6%) without apparent stenosis were selected as control group. The clinical features were compared between two groups. [Results] Univariate analysis showed that there were significant differences in the degree of hypertension, lipid metabolism disturbance and ischemic heat disease between two groups( P 〈0.05). Multiple logistic regression analysis showed that hypertension, diabetes mellitus and lipid metabolism disturbance were the independent risk factors of intracranial artery stenosis. [Conclusion] Hypertension, diabetes mellitus and lipid metabolism disturbance may be the independent risk factors which lead to intracranial artery stenosis. It must be further confirmed by large sample of cohort study.
