不同剂量米非司酮治疗子宫肌瘤的临床观察

目的 比较3种不同剂量米非司酮治疗子宫肌瘤的临床效果。方法 将49例子宫肌瘤患者随机分为3组。A组21例，每日口服米非司酮10mg；B组16例，每日口服米非司酮12．5mg；C组12例，每日口服米非司酮25mg。从月经周期第2天开始服药，3个月为1疗程，治疗前及治疗期间每月用B超测量子官及肌瘤体积；检查血红蛋白及肝、肾功能。结果 3组患者在治疗期间均出现闭经，症状消失，血红蛋白恢复正常，肝、肾功能无明显变化。3组子宫及肌瘤体积均明显缩小，A组肌瘤缩小了44．9％，B组肌瘤缩小了46．0％，C组肌瘤缩小了47．4％。3组之间差异无显著性。结论 米非司酮是一种治疗子宫肌瘤的有效药物，每日口服米非司酮10mg可能是较为理想的治疗剂量。

Clinical observation of different dose of mifepristone in treating uterine leiomyoma

Objective To evaluate the response of uterine leiomyoma to three different daily dosages of mifepristone. Methods Forty nine patients with uterine leiomyoma were divided into three groups randomly. Twenty one patients were treated with 10mg of mifepristone daily, sixteen patients with 12.5mg daily and twelve patients with 25mg daily for 3 months, beginning on day 2 of the menstrual cycle. Volume changes of leiomyoma and uterus were measured with ultrasonography before treatment and monthly during the course of therapy. Blood samples for hemoglobin, liver and renal function were obtained monthly. Results Ovarine acyclicity was induced in 3 groups of patients given different daily dose of mifepristone and clinical symptoms improved obviously. After 3 months of mifepristone treatment, the leiomyoma volume decreased to 39.8% of baseline in group A, 40.5% in group B and 44.4% in group C. There was no significant difference in the three treatment groups.Conclusion Mifepristone is a practical and hopeful drug in treating uterine leiomyoma, 10 mg of mifepristone daily is considered to be a more ideal dosage for treatment.
