非小脑蚓部髓母细胞瘤的DWI及动态增强MR表现

摘    要:目的 探讨非小脑蚓部髓母细胞瘤(MB)的DWI及动态增强MRI表现。方法 回顾性分析23例经病理证实的非小脑蚓部MB患者的DWI及动态增强MR图像。分别测量病灶ADC值(ADCMB)和相同层面脑白质ADC值(ADC白质),获取时间-信号强度曲线(TIC)。计算并比较各类型TIC的达峰时间(Tmax)、增强峰值(EP)、最大对比增强率(MCER)。结果 MB的DWI均呈相对高信号,ADC呈相对低信号。TIC曲线类型为流入型4例(4/23,17.39%),平台型19例(19/23,82.61%)。流入型Tmax为(112.33±8.33)s,平台型Tmax为(81.18±13.12)s,差异有统计学意义(t=3.84,P=0.02)。结论 DWI和动态增强MRI对诊断非小脑蚓部MB有重要临床价值。

Abstract:Objective To explore the characteristics of DWI and dynamic contrast-enhanced MRI in medulloblastoma (MB) located out of cerebellar vermis.Methods MRI and DWI data of 23 patients with MB located out of cerebellar vermis confirmed by pathology were retrospectively studied.ADC of the lesions (ADCMB) and the white matter of the same slice (ADCwhite matter) was measured and compared.Time-intense curve (TIC) were obtained.Peak time (Tmax),enhancement peak (EP) and maximum contrast enhancement ratio (MCER) of different TIC types were compared and analyzed.Results DWI of MB showed higher signal,and ADC displayed low signal.Four cases (4/23,17.39%) showed inflow type of TIC,and 19 cases (19/23,82.61%) showed platform type of TIC.The difference of Tmax between inflow and platform type was statistically significant,the Tmax of inflow type was higher than that of platform type (112.33±8.33]s vs 81.18±13.12]s;t=3.84,P=0.02).Conclusion DWI and dynamic contrast-enhanced MRI in diagnosing MB located out of cerebellar vermis has great clinical value.
