小脑顶核电刺激对阿尔茨海默病磁共振波谱代谢物的影响

摘    要:目的 采用MRS评价小脑顶核电刺激(FNS)对阿尔茨海默病(AD)患者后扣带回(PCC)代谢物的影响。方法 收集AD患者36例,随机分为治疗组和安慰组,各18例。治疗组给予小脑FNS治疗。治疗前、后采用MRS评估PCC代谢产物N-乙酰天门冬氨酸(NAA)/肌酸(Cr)值、肌醇(mI)/Cr值。比较两组治疗前、后代谢物的差异。结果 治疗组治疗后NAA/Cr值较治疗前轻度升高,但差异无统计学意义(P>0.05),mI/Cr值较治疗前降低,差异有统计学意义(t=-2.502,P<0.05);治疗后治疗组NAA/Cr值较安慰组升高,mI/Cr值降低,差异均有统计学意义(t=4.176、-4.987,P均<0.05)。结论 FNS治疗后AD大脑代谢物发生了变化,MRS可作为观察AD的客观辅助手段。

Abstract:Objective To assess the metabolite alteration of posterior cingulated cortex (PCC) in Alzheimer disease (AD) patients after therapy of fastigial nucleus electrostimulation (FNS) with MRS.Methods Thirty-six AD patients were randomly divided into treatment group and control group (each n=18).FNS was used to the treatment group.The ratios of N-Acetyl L-Aspartic acid (NAA)/Creatine (Cr) and Myo-inositol (mI)/Cr of PCC were assessed with MRS.The differences of metabolites in two groups before and after treatment were compared.Results Post-therapeutic ratio of NAA/Cr in treatment group increased slightly (P>0.05),mI/Cr was lower than the pre-therapeutic (t=-2.502,P<0.05).Compared with control group,NAA/Cr in treatment group increased,mI/Cr decreased,which were statistically significant differences (t=4.176,-4.987,all P<0.05).Conclusion The brain metabolites are altered in patients with AD after FNS.MRS is a useful and objective tool for observing AD.
