超声刀与传统手术行甲状腺全切除术的疗效观察

目的探讨超声刀与传统手术在甲状腺全切除术中的应用效果。方法选取2018年2月至2019年5月间上海杨浦区市东医院收治的88例恶性甲状腺癌和良性甲状腺疾病患者,采用计算机程序随机分为超声刀组和传统手术组,每组44例。超声刀组患者采用超声刀治疗,传统手术组患者采用常规电刀治疗,比较两组患者手术时间、切口长度、术中出血量、术后引流时间、术后引流量和住院时间等手术情况及术后并发症发生情况。结果超声刀组患者手术时间、切口长度、术中出血量、术后24 h引流量、术后总引流量、术后引流时间和住院时间均较传统手术组低,差异均有统计学意义(均P<0.05)。超声刀组患者术后并发症总发生率为4.5%,低于传统手术组的15.9%,差异有统计学意义(P<0.05)。结论采用超声刀行开放式甲状腺全切除术安全有效,临床疗效和安全性较电刀更好,值得临床推广应用。

Efficacy of total thyroidectomy with ultrasonic scalpel versus conventional surgery

Objective To investigate the efficacy of total thyroidectomy with ultrasonic scalpel versus conventional surgery.Methods This study included 88 patients with malignant thyroid cancer and benign thyroid disease who were treated at Shidong Hospital of Yangpu District from February 2018 to May 2019.They were randomly divided into an ultrasonic scalpel group and a conventional surgery group with ultrasonic knife and conventional electric scalpel,respectively with 44 patients in each group.Surgical conditions such as duration of operation,incision length,intraoperative bleeding volume,duration of postoperative drainage,postoperative drainage volume,length of hospital stay and incidence of postoperative infections were compared between the two groups.Results,The duration of operation,incision length,intraoperative blood loss,postoperative drainage volume at 24 h after the operation,the overall drainage volume after the operation and length of hospital stay was lower in the ultrasonic scalpel group than in the conventional surgery group(all P<0.05).Complications occurred in 2 patients(4.5%)in the ultrasonic knife group which was lower than 7 patients(15.9%)in the control group(P<0.05).Conclusion Total thyroidectomy with ultrasonic scalpel is safe and effective whose clinical efficacy and safety is superior to electric scalpel and it is worthy of clinical application.
