乳酸检测在新生儿窒息后脑损害中的应用价值

摘    要:目的探讨出生窒息新生儿入院24 h内动脉血乳酸检测在新生儿窒息后脑损害中的应用价值。方法选取2017年8月—2019年8月福州市第一医院NICU收住的出生窒息新生儿180例为研究对象,研究对象不包括早产儿、足月小样儿。其中男患儿92例,女患儿88例,胎龄均≥37周,出生体质量均≥2500 g。将其分为无脑损害组101例及有脑损害组79例,比较两组入院24 h内动脉血乳酸水平。同时将脑损害组分为轻-中度脑损害组61例及重度脑损害组18例,比较两组入院24 h内动脉血乳酸水平。结果新生儿窒息后动脉血乳酸水平均有不同程度的增高,有脑损害组动脉血乳酸水平高于无脑损害组,差异具有统计学意义(P<0.05)。重度脑损害组血乳酸水平高于轻-中度脑损害组,差异具有统计学意义(P<0.05)。结论动脉血乳酸检测可以作为判断新生儿窒息后脑损害的一个良好指标,为出生窒息新生儿脑损害判断及预后评估提供了研究价值。

Application Value of Lactic Acid Detection in Brain Damage After Neonatal

Abstract:Objective To explore the application value of arterial blood lactate test for cerebral damage after neonatal asphyxia 24 hours after admission.Methods 180 cases of asphyxia neonates admitted to NICU in Fuzhou first hospital from August 2017 to August 2019 were selected as the study subjects,excluding preterm and full-term infants.There were 92 males and 88 females,with gestational age≥37 weeks and birth weight≥2500 g.The patients were divided into 101 patients without brain damage and 79 patients with brain damage.The arterial blood lactic acid levels within 24 hours after admission were compared between the two groups.At the same time,the brain injury group was divided into 61 patients with mild-moderate brain damage and 18 patients with severe brain damage.The arterial blood lactic acid levels within 24 hours after admission were compared between the two groups.Results After neonatal asphyxia arterial blood lactate levels increased to some extent,have brain damage arterial blood lactic acid level is higher than no brain damage group,the difference was statistically significant(P<0.05),severe brain damage to blood lactic acid level is higher than mild-moderate brain damage group,the difference was statistically significant(P<0.05).Conclusion The detection of arterial blood lactic acid can be used as a judgment of neonatal asphyxia after brain damage a good indicator for birth asphyxia newborn brain damage judgment and prognosis assessment provides the research value.
