布托啡诺超前镇痛对人工流产术后疼痛的影响

目的观察布托啡诺术前静脉注射对人工流产静脉麻醉及术后子宫收缩疼痛的影响。方法采用双盲法随机将80例早期妊娠妇女分成试验组（A组）和对照组（B组）,每组各40例。术前5m in,试验组静脉注射布托啡诺20μg/kg,对照组静脉注射安慰剂（生理盐水）。两组均以芬太尼复合丙泊酚静脉麻醉。观察两组诱导时间、苏醒时间、丙泊酚用药总量、呼吸循环参数改变、术后疼痛评分以及补救镇痛药情况。结果术后疼痛评分A组显著低于B组（P〈0.05）,补救镇痛药在B组使用比例更高（P〈0.05）。术中丙泊酚用药总量A组少于B组（P〈0.05）。两组呼吸循环抑制发生率及诱导、苏醒时间无显著性差异（P〉0.05）。结论布托啡诺超前镇痛用于无痛人工流产可增强麻醉效果,减少丙泊酚用量,减少术后腹痛的发生率,可明显缓解术后子宫收缩疼痛。

The preemptive analgesia of Butorphanol in painless induced abortion

Objective To evaluate preemptive analgesia of butorphanol in painless induced abortion. Method 80 female patients undergoing painless abortion were randomized two groups： butorphanol group （ group A, n = 40）, control group （ group B, n = 40）. Patients in group A were received butorphanol 20μg/kg at 5 minutes before operation, patients in group B were received placebo at the same time. All the patients were given prepofol and fentanyl for anesthesia. Changes of vital sign and prepofol doses, post - operation revival time, visual analog scale （VAS） and side effects were observed. Results There were no difference in blood pressure.heart rate.SPO2and the perceptive time.postoperative nausea and vomiting also were no difference in two groups （ P 〉 0. 05 ）. The group A and B showed a significant decrease in the total dosage of prepofol and VAS （ P 〈 0. 05 ）. Conclusion Butorphanol used in painless induced abortion can significantly decrease the total dosage of prepofol and the pain intensity of lower abdominal after operationl.
