副神经节瘤的细胞学诊断及鉴别诊断

目的探讨副神经节瘤的细胞学特点及鉴别诊断。方法 4例副神经节瘤病例分别来自穿刺细胞学2例,术中刮片细胞学2例。行常规HE染色,光镜观察其细胞学特点,并与涎腺肌上皮瘤、小细胞性神经内分泌癌、非霍奇金氏淋巴瘤和恶性黑色素瘤等细胞学特点进行比较。结果 4例涂片细胞学量较丰富,以较单一的松散性排列的细胞为主,可见少量胞质或无胞质,细胞核呈卵圆形或长梭形,染色质细颗粒状,可见小核仁,偶见明显增大的核仁,胞质内包涵体少见,刮片细胞学中可见分枝状的血管腔隙样结构,长杆状支持细胞偶见。而浆细胞样肌上皮瘤多呈胞质较宽的单一性细胞散在性分布,非霍奇金氏淋巴瘤瘤细胞呈无粘连性单一性增生,恶性黑色素瘤多表现为明显增大的嗜酸性核仁,小细胞神经内分泌癌细胞学涂片背景可见明显凝固性坏死,与副神经节瘤易于区分。结论副神经节瘤的细胞学涂片有其独特特点,细胞学检查有助于副神经节瘤的术前或术中诊断,但发生在血管旁的副神经节瘤需严格掌握其穿刺细胞学适应征。

Fine needle aspiration cytology of paraganglioma and its differential diagnosis
