甲苓饮治疗阴虚型肝硬化腹水的临床研究

观察中药制剂甲苓饮治疗阴虚型肝硬化腹水的临床疗效。方法：将所选病例随机分成两组，其中治疗组60例，对照组30例。治疗组患者口服甲苓饮，对照组以对症（利尿剂）、支持（白蛋白、血浆）或加用保肝药物（如阿拓莫兰），3个月为1个疗程。治疗前后观察患者腹围、体重、肝功能（ALT、TBil、Alb）、凝血酶原活动度（PTA）、肝纤维化指标（HA、LN、PCⅢ、CⅣ）、上腹部彩色B超等检查。结果：两组患者治疗后在症状积分、ALT、PTA、HA、LN、PCⅢ、C1V方面差异有显著性意义（P〈0、01）；在TBil、Alb、腹围、体重及肝门静脉压方面差异亦有显著性意义（P〈0．05）；在脾厚值方面比较差异无显著性意义（P〉0．05）。结论：中药制剂甲苓饮能明显改善患者的症状、体征、肝门静脉压、肝功能、凝血酶原活动度及血清肝纤维化指标，疗效较对照组明显。

Effect of TCM Jia Ling Yin on type Yin deficiency ascites due to hepatocirrhosis

Objective： To observe the curative effect of the TCM ＂Jia Ling Yin＂ on the type Yin deficiency ascites caused by hepatocirrhosis. Methods： Ninety cases of type Yin deficiency ascites were divided into two groups at random： the treatment group was 60 cases and the control group was 30 cases. The treatment group was treated with TCM ＂Jia Ling Yin＂ orally, while the control group was treated with the medicine according to the syndrome （emictory） and sustained with albumin and blood plasma or treated with the drugs to nourish the liver, such as ＂Reduced Glutathione for Injection＂ . And a period of treatment lasted for three months. Before and after the treatment, the patient＇s abdominal perimeter, body weight, liver function, pro- thrombin activity, liver fibrosis indexes and colorful ultrasonic diagnosis B of the upper abdomen were observed. Results： There was a striking difference between the two groups on the symptom credits, ALT, PTA, HA, LN, PCⅢ, CⅣ （P 〈0. 01） , TBil, Alb, abdominal perimenter, body weight and the pressure of hepatic portal （P 〈 0. 05） . However, there was no significant difference between the patients on the thickness of spleen （P 〉 0. 05 ） . Conclusion： TCM ＂Jia Ling Yin＂ can distinctively improve the patients＇ symptom, physical signs, portal venous pressure, liver function, prothrombin activity and serum liver fibrosis indexes. And the curative effect of the TCM ＂Jia Ling Yin＂ on the treatment group is obvious in comparison with the control group.
