心肌梗死恢复期运动试验和动态心电图检查的意义

37例心肌梗死恢复期患者作运动试验(ET)和动态心电图(AECG)检查,ET 对 ST 段压低的检出率(27%)显著高于 AECG(13.5%);对多支病变两项检查皆有诊断价值;5例两项均有 ST 段压低者3例发生梗死后频发心绞痛,显著多于无 ST 段压低者(P=0.008)。2例发生梗死后急性左心衰者为室壁瘤伴 ET 中 ST 段抬高者,室壁瘤患者两项检查中室性心律失常显著增多。

Predictive value of exercise testing and ambulatory electrocardiographic monitoring after acute myocardial infarction

37 patients with acute myocardial infarction underwent both exercise testing(ET)and ambulatory electrocardiographic monitoring(AECG)in recovery period.The incidence ofST segment depression in ET(27%)was significantly higher than that in AECG(13.5%).ST seg-ment depression both in ET or in AECG had predictive value for detection of multivessel disease.3 of 5 patients with ST segment depression both in ET and in AECG developed frequent postin-farction angina pectoris,which was significantly higher than the patients without ST segment de-pression both in ET and AECG(P=0.008).2 patients with both aneurysm and ST segment ele-vation in ET developed acute left ventricular failure during follow-up.The patients with a-neurysm had significantly more cases with venricular arrhythmia in ET or in AECG than the pa-tients without aneurysm.
