| 摘 要： | 目的 临床药师通过参与1例慢性肾炎患者的药学监护,探讨在临床实践中临床药师如何发挥作用.方法 临床药师协助医师积极查找病因,根据患者病情变化,提出药物治疗方案的建议,对患者进行药学监护和用药教育.结果 临床药师提供的药学服务,对明确病因、提高患者用药依从性、使患者尽早康复提供了有益的帮助.结论 临床药学服务可为合理用药提供有力的帮助.

| Abstract: | Objective To discuss the role of clinical pharmacists in clinical practice through the pharmaceutical care for 1 patient with chronic glomerulonephritis.Methods The clinical pharmacists actively assisted physicians in finding out the etiology,putting forward therapeutic schemes,and carrying out pharmaceutical care and education for the patient.Results Pharmaceutical care from clinical pharmacists for patients with chronic glomerulonephritis was helpful for clarifying the etiology,improving patient adherence and healing.Conclusion Clinical pharmacy service can provide effective help in rational drug use. |
