锁定加压钢板在股骨骨折中的临床应用

目的研究锁定加压钢板在股骨骨折中的临床应用效果,为股骨外科治疗提供一定的理论依据。方法选择2007年6月至2010年6月期间在我科诊治的股骨骨折患者78例,按照手术方法分成2组,每组39例,其中对照组患者采用动力髋螺钉固定治疗,研究组患者均采用锁定加压钢板固定治疗,治疗后对两组患者的相关临床资料进行比较分析。结果两组患者在住院时间和愈合时间方面具有统计学差异,且研究组的住院时间和愈合时间与对照组相比较显著缩短（P〈0.05）;在治疗前,两组患者之间的VAS疼痛评分没有统计学差异（P〉0.05）,而在治疗12 h、3 d、7 d后,两组患者的VAS疼痛评分较治疗前具有显著的统计学差异（P〈0.05）;随访3个月时,对照组的总有效率为69.23%,研究组的总有效率为89.74%,相比较两组的总有效率具有统计学差异（P〈0.05）;当随访6个月时,两组的总有效率还是具有统计学差异（P〈0.05）,对照组和研究组分别为82.05%和100%。结论锁定加压钢板内固定治疗股骨骨折较动力髋螺钉内固定效果更显著,而且具有住院时间短和骨折恢复快的优势。

Clinical application of locking compression plate in the treatment of femoral fractures

Objective To investigate the clinical effects of locking compression plate on the treatment of femoral fractures and to provide related theoretical references.Methods Totally 78 patients with distal femoral fractures were selected and divided into control group and study group（n=39）.The patients in control group were took dynamic hip screw fixation while those in study group were received locking compression plate fixation.The clinical data of the two groups were compared after the treatment.Results These was no significant difference in operative blood loss and operative time between the two groups（P〉0.05）,but there were significant differences in hospitalization day and fracture healing time between the two groups（P〈0.05）.There was no difference in VAS score before the treatment between the two groups（P〉0.05）.Significant differences in VAS score were observed at 12 hours,3 days and 7 days after the treatment（P〈0.05） compared with those before the treatment,but there was no difference between the groups at 12 hours,3 days and 7 days after the treatment（P〉0.05）.These were significant differences in the total effective rate between the two groups,69.23% vs.89.74% at 3 months,82.05% vs.100% at 6 months（P〈0.05）.Conclusion With the advantages of shorter hospitalization and quicker recovery,locking compression plate fixation is effective in treating distal femoral fractures.
