不同剂型当归拈痛汤治疗类风湿关节炎的临床疗效比较

目的 :观察不同剂型当归拈痛汤治疗活动期类风湿关节炎(RA)的临床疗效及对血清中白介素-1(IL-1)、白介素-6(IL-6)、肿瘤坏死因子α(TNF-α)的影响。方法 :将90例活动期类风湿关节炎患者随机分为A、B、C 3组,每组30例,A组给予当归拈痛汤传统汤剂,B组给予当归拈痛汤丸剂,C组给予当归拈痛汤颗粒剂。3组患者同时口服甲氨蝶呤治疗。治疗3个月后观察临床疗效对及对血清中IL-1、IL-6、TNF-α的影响。结果:治疗后A组总有效率70.00%,B组总有效率93.33%,C组总有效率73.33%。3组治疗前后血清IL-1、IL-6、TNF-α的比较有统计学意义(P0.05),治疗后B组较A、C组的血清IL-1、IL-6、TNF-α明显降低,差异有统计学意义(P0.05)。结论:当归拈痛汤丸剂治疗活动期类风湿关节炎的疗效显著优于传统汤剂和颗粒剂,能明显降低活动期类风湿关节炎血清中IL-1、IL-6、TNF-α水平,提高临床疗效。

Comparison of Clinical Efficacy of Danggui Niantong Decoction of Different Dosage Form in Treatment of Rheumatoid Arthritis

Objective:To observe the clinical efficacy of Danggui Niantong decoction of different dosage in the treatment of active rheumatoid arthritis(RA) and the effect on serum interleukin-1(IL-1),interleukin-6(IL-6),tumor necrosis factor-α(TNF-α). Methods:There were 90 patients with active RA randomly divided into A,B and C groups,30 cases in each group. Group A received the traditional Danggui Niantong decoction,group B received Danggui Niantong decoction in pill forms,and group C received Danggui Niantong decoction in granule forms. Also patients in the 3 groups were given oral administration of methotrexate. After3 months of treatment,the clinical effect and the levels of serum IL-1,IL-6 and TNF-α were observed. Results:After the treatment,the total effective rate in group A was 70%,in group B was 93.33%,and in group C was 73.33%. Before and after treatment,changes on the levels of serum IL-1,IL-6 and TNF-α were statis tically significant(P<0.05). The levels of serum IL-1,IL-6 and TNF-α in group B were significantly lower than those in the group A and group C,and the difference were statistical significant(P<0.05). Conclusion:The curative effect of Danggui Niantong decoction of pill form for the treatment of active RA was significantly better than the traditional decoction and granule form,for it can obviously reduce serum IL-1,IL-6 and TNF-α levels,and improve the clinical efficacy.
