加味半夏白术天麻汤治疗急性缺血性脑卒中疗效观察

目的:观察加味半夏白术天麻汤治疗急性缺血性脑卒中的临床疗效。方法:140例风痰阻络证型急性缺血性脑卒中病例随机分为观察组和对照组各70例。对照组给予常规西医治疗,观察组在对照组的基础上给予加味半夏白术天麻汤治疗。治疗2周后,观察两组疗效及血清炎症因子(IL-6、hs-CRP、TNF-α)及凝血功能指标(PT、TT、APTT、FIB)的变化。结果:(1)临床疗效,观察组基本痊愈率42.86%、显著进步率38.51%、进步率14.29%;对照组分别为27.14%、32.86%、24.29%,两组疗效比较差异有统计学意义(P0.05);(2)血清炎症因子水平,两组IL-6、hs-CRP、TNF-α水平比较差异有统计学意义(P0.05),且两组IL-6、hs-CRP、TNF-α水平随着时间的延长逐渐降低(P0.05),观察组下降的幅度明显大于对照组(P0.05);(3)凝血功能,两组PT、APTT、TT、FIB水平比较差异有统计学意义(P0.05),两组PT、APTT、TT水平随着时间的延长逐渐升高(P0.05),而FIB逐渐降低(P0.05),观察组变化幅度大于对照组(P0.05)。结论:加味半夏白术天麻汤可显著提高急性缺血性脑卒中的临床疗效,作用机制可能与抑制炎症反应、改善凝血功能有关。

Observation of Efficacy of Modified Banxia Baizhu Tianma Decoction on Treating Acute Ischemic Stroke

Objective:To observe the efficacy of Jiawei Banxia Baizhu Tianma decoction on treating patients with acute ischemic stroke. Methods:A total of 140 patients with acute ischemic stroke of wind and phlegm blockage channel syndrome were randomly divided into the observation group and the control group,with 70 cases in each group. The control group was given conventional western medicine treatment,and the observation group was given Jiawei Banxia Baizhu Tianma decoction besides the treatment in the control group. After 2 weeks,the clinical efficacy and changes of serum inflammatory factors(IL-6,hs-CRP,TNF-α) and coagulation function(PT,TT,APTT,FIB) of the two groups were observed. Results:(1)Comparison of clinical efficacy:in the observation group and the control group,the general cure rates were 42.86% and 27.14%,the significant improvement rates were 38.51% and 32.86%,the improvement rates were 14.29% and 24.29%,respectively. The difference was statistically significant(P<0.05);(2)Comparison of inflammatory factors levels:There was significant difference on levels of IL-6,hs-CRP,TNF-α in these two groups(P<0.05),all of them decreased gradually with time increasing(P<0.05),and the decrease in the observation group was greater than that in the control group(P <0.05);(3) Comparison of coagulation function levels:There was significant difference of PT,APTT,TT,FIB in the two groups(P<0.05),PT,APTT and TT increased with time increasing(P<0.05),while FIB decreased(P<0.05);the change in the observation group was greater than that in the control group(P<0.05). Conclusion:Jiawei Banxia Baizhu Tianma decoction can effectively improve the clinical efficacy on acute ischemic stroke,and its mechanism may be related to inhibiting inflammatory reaction and improving coagulation function.
