Application value of high frequency color Doppler ultrasound combined with GGT in infantile biliary atresia

Objective:To investigate the diagnostic value of high-frequency color Doppler ultrasound combined with r-glutamyl transpeptidase(GGT)in infantile biliary atresia.Methods:A retrospective analysis of 95 children with non-infectious cholestasis and jaundice,including 80 cases of biliary atresia and 15 cases of non-biliary atresia,all children were confirmed by histopathological biopsies.Indicators of liver function and high-frequency ultrasound in each group were collected and compared for accuracy,sensitivity,specificity,positive predictive value and negative predictive value.Results:The accuracy,sensitivity,specificity,positive predictive value and negative predictive value in different conditions were shown below:the diagnosis of biliary atresia with GGT>800 IU/L were 85%,90%,60%,92%and 53%,respectively.Positive blood flow under the liver capsule in diagnosing biliary atresia were 83%,87%,60%,92%and 47%,respectively.The patients with abnormal gallbladder were 87%,92%,67%,93%and 58%,respectively.Combined diagnosis of gallbladder abnormality+GGT for biliary atresia were 89%,92%,73%,94%and 65%,respectively.Combined diagnosis of gallbladder abnormality+positive subhepatic blood flow in the diagnosis of biliary atresia were 92%,94%,80%,96%and 71%,respectively.Abnormal gallbladder+positive subhepatic blood flow+GGT were 93%,94%,93%,98%and 74%,respectively.The accuracy and specificity of pairwise combination were higher than that of single index,and the negative predictive value was higher than that of positive predictive value.Conclusion:High frequency color Doppler examination combined with GGT is an effective supplement for screening biliary atresia in addition to routine two-dimensional ultrasound examination,which provides a favorable basis for the diagnosis and differential diagnosis of biliary atresia.
